抑郁症患者血清脂蛋白相关磷脂酶A2水平变化及其临床意义

摘　　要：目的 探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)在抑郁症中的临床意义。方法 选取2022年1月—2023年12月同济大学附属精神卫生中心抑郁症患者120例(抑郁症组)和健康体检者120名(正常对照组)。评估所有抑郁症患者病情,采用健康问卷抑郁自评量表(PHQ)-9和汉密尔顿抑郁量表(HAMD)-17评估其抑郁程度,根据病情严重程度分为轻度组、中度组和重度组。收集所有研究对象一般资料,并检测血清Lp-PLA2、去甲肾上腺素(NE)、多巴胺(DA)和5-羟色胺(5-HT)水平。采用Pearson相关分析评估各指标之间的相关性。采用Logistic回归分析评估血清Lp-PLA2对抑郁症病情严重程度的影响。结果 抑郁症组血清Lp-PLA2水平显著高于正常对照组(P<0.05)。轻度组、中度组和重度组PHQ-9评分、HAMD-17评分、血清Lp-PLA2水平依次升高(P<0.05),血清NE、DA、5-HT水平依次降低(P<0.05)。抑郁症患者血清Lp-PLA2与PHQ-9评分、HAMD-17评分呈正相关(r值分别为0.424、0.325,P <0.05),与血清NE、DA、5-HT水平呈负相关(r值分别为–0.387、–0.301、–0.338,P<0.05)。血清Lp-PLA2水平升高是抑郁症病情进展的危险因素[比值比(OR)=1.039,95%可信区间(CI)为1.024~1.054,P<0.001)。结论 血清Lp-PLA2水平与抑郁症患者病情进展有关,或可作为抑郁症病情评估的指标。Objective To investigate the clinical significance of serum lipoprotein-associated phospholipase A2(Lp-PL A2)in depression.Methods A total of 120 patients with depression(depression group)and 120 healthy subjects(healthy control group)were enrolled from the Mental Health Center Affiliated to Tongji University from January 2022 to December 2023.All the patients with depression were evaluated by patient health questionnaire(PHQ)-9 and Hamilton depression scale(HAMD)-17.They were classified into mild group,moderate group and severe group according to the severity of depression.The general data of all the subjects were collected,and serum Lp-PLA2,norepinephrine(NE),dopamine(DA)and 5-hydroxytryptamine(5-HT)levels were determined.Pearson correlation analysis was used to evaluate the correlation between indicators.Logistic regression analysis was used to evaluate the effect of serum Lp-PLA2 on the severity of depression.Results The serum Lp-PLA2 level in depression group was higher than that in healthy control group(P<0.05).PHQ-9 scores,HAMD-17 scores and serum Lp-PLA2 levels in mild,moderate and severe groups were increased successively(P<0.05),while serum NE,DA and 5-HT levels were decreased successively(P<0.05).Serum Lp-PLA2 was positively correlated with PHQ-9 score and HAMD-17 score(r values were 0.424 and 0.325,P<0.05),and it was negatively correlated with serum NE,DA and 5-HT(r values were–0.387,–0.301 and–0.338,P<0.05).The increased serum Lp-PLA2 level is a risk factor for the progression of depression[odds ratio(OR)=1.039,95%confidence interval(CI)1.024-1.054,P<0.001].Conclusions The serum Lp-PLA2 level is related to the disease progress of depression patients,and it may be used as one of the indicators for the evaluation of depression.
