摘 要:目的探讨脊柱外科手术患者麻醉持续时间与术后跌倒风险的关系。方法选择2020年1月至2023年6月在该院全身麻醉下脊柱手术后住院期间发生跌倒事件的37例患者作为研究组,按照1:3的比例选择111例无跌倒患者作为对照组。记录患者的性别、年龄、婚姻状况、文化程度、伴随疾病、服用药物类型、视力状况、眩晕状况、睡眠状况、大小便、美国麻醉协会(ASA)分级、手术类型、手术节段、全身麻醉持续时间、手术时间等资料。比较两组患者临床资料差异,采用多因素Logistic回归分析全身麻醉持续时间与跌倒的关系。结果研究组全身麻醉时间长于对照组(P<0.05)。两组患者性别、年龄、高血压、降压药、视力、听力、睡眠状况、小便、ASA分级、手术节段、手术时间的差异存在统计学意义(P<0.05)。多因素Logistic回归分析结果显示,全身麻醉时间不是影响术后跌倒的风险因素(P>0.05),影响术后跌倒的风险因素包括年龄≥75岁、ASA>2级和男性(P<0.05)。结论全身麻醉时间不影响全身麻醉脊柱手术后跌倒的风险,影响全身麻醉脊柱手术后跌倒的风险因素包括男性、年龄≥75岁、ASA分级>2级。Objective To explore the relationship between the duration of anesthesia and the risk of postoperative falls in patients undergoing spinal surgery.Method 37 patients who experienced falls during hospitalization after spinal surgery under general anesthesia in our hospital from January 2020 to June 2023 were selected as the study group,and 111 patients without falls were selected as the control group in a ratio of 1∶3.The patients'gender,age,marital status,educational level,comorbidities,type of medication taken,visual status,dizziness,sleep status,bowel movements,American Society of Anesthesiology(ASA)classification,surgical type,surgical segment,duration of general anesthesia,and surgical time were recorded.The difference in clinical data between two groups of patients was compared,and a multivariate logistic regression analysis was used to investigate the relationship between the duration of general anesthesia and falls.Results The total anesthesia time in the study group was higher than that in the control group(P<0.05).There were statistically significant differences between the two groups of patients in terms of gender,age,hypertension,antihypertensive drugs,vision,hearing,sleep status,urination,ASA grading,surgical segment,and surgical time(P<0.05).The results of multivariate logistic regression analysis showed that the duration of general anesthesia was not a risk factor for postoperative falls(P>0.05).The risk factors for postoperative falls included age≥75 years,ASA>grade 2,and male(P<0.05).Conclusion The duration of general anesthesia does not affect the risk of falls after spinal anesthesia surgery.The risk factors for falls after spinal anesthesia surgery include male gender,age≥75 years old,and ASA grade>Grade 2.
