单侧双通道内镜技术治疗腰椎退变性疾病的疗效观察

摘　　要：目的探讨单侧双通道内镜技术(UBE)治疗腰椎退变性疾病的疗效。方法回顾性分析2021年1月~2022年7月安庆市立医院骨科使用单侧双通道内镜技术(UBE)治疗的49例腰椎退变性疾病患者资料。记录手术时间、术中出血量、术后引流量、术后住院时间等,评估其安全性;比较患者术前、术后3 d、术后3个月及术后6个月时的腰痛视觉模拟评分(VAS)、腿痛VAS评分、Oswestry功能障碍指数(ODI),评估其临床疗效。结果49例患者均顺利完成手术,术后1例出现脑脊液漏,1例出现感染,予以相关处置后均治愈。随时间推移,术后患者腰痛VAS评分、腿痛VAS评分、ODI指数较术前显著持续下降,差异有统计学意义(P<0.05)。按改良Macnab标准评价疗效:优32例,良11例,可6例,优良率为87.76%。结论单侧双通内镜技术是治疗腰椎退变性疾病安全可靠的方法。Objective To investigate the efficacy of unilateral biportal endoscopy(UBE)in the treatment of lumbar degenerative diseases.Methods 49 patients with lumbar degenerative diseases(including lumbar disc herniation and lumbar spinal stenosis)treated with UBE in Department of Orthopedics,Anqing Municipal Hospital from January 2021 to July 2022 were retrospectively analyzed.Clinical date such as operation time,intraoperative blood loss,postoperative drainage,and postoperative hospital stay were recorded to evaluate its safety.VAS for low back pain and lower extremity pain and ODI index were compared before operation,3 days,3 months and 6 months after operation to evaluate the clinical efficacy.Results 49 patients with lumbar degenerative diseases were successfully operated by unilateral biportal endoscopy and followed up completely.One case had cerebrospinal fluid leakage and one case had infection occurred after operation,which were cured after relevant treatment.As time went by,VAS for low back pain and lower extremity pain and ODI index decreased significantly and continuously compared with those before operation,and the difference was statistically significant(P<0.05).The modified Macnab criteria suggested that 32 cases were excellent,11 cases were good,6 cases were fair and the excellent and good rate was 87.76%.Conclusion Unilateral biportal endoscopy is a safe and reliable method for the treatment of lumbar degenerative diseases.
