揿针技术在老年髋部骨折患者术后护理中的应用效果

摘　　要：目的:探究揿针技术在老年髋部骨折患者术后护理中的应用效果。方法:选取2021年2月至2022年11月宁波市第六医院收治的236例老年髋部骨折术后患者作为研究对象,120例患者采用常规护理措施(常规护理组),116例患者根于辩证施护为气虚血瘀型给予揿针埋针护理5 d(揿针护理组)。对比两组患者疼痛程度(VAS评分)、睡眠质量、Harris评分及不良反应发生率的差异,通过二元Logstic回归分析和多重线性回归分析分别筛选影响患者VAS评分及Harris评分的影响因素。结果:术后1、3、5 d揿针护理组患者VAS评分均低于常规护理组(P<0.05);术后3、5 d揿针组患者睡眠质量评分均优于常规护理组睡眠质量评分(P<0.05)。术后1周揿针护理组患者Harris评分高于常规组(P<0.05);揿针组患者便秘、恶心呕吐发生率均低于常规护理组(P<0.05)。二元Logstic回归分析显示揿针护理是术后1 d VAS评分严重程度的影响因素(OR=0.249,95%CI=0.186~0.609,P<0.001);多重线性回归分析显示揿针护理是术后1周Harris评分的影响因素(β=1.324,95%CI=1.201~2.668,P<0.001)。结论:在老年髋部骨折患者术后护理中应用揿针护理,能早期缓解术后疼痛,改善患者髋关节功能,降低恶心呕吐、便秘等不良反应发生率。Objective:To explore the application effect of press-needle technique in the postoperative nursing of elderly patients with hip fracture.Methods:A total of 236 elderly patients with hip fracture admitted to Ningbo No.6 Hospital from February 2021 to November 2022 were selected as the research subjects,of whom 120 patients were included in the routine nursing group who were given routine nursing measures and 116 were in the press-needle nursing group who were given needling nursing for 5 days based on dialectic nursing for qi deficiency and blood stasis type.The differences in pain degree(VAS score),sleep quality,Harris score and incidence of adverse reactions were compared between the two groups.Binary Logstic regression analysis and multiple linear regression analysis were used to screen the influencing factors affecting the VAS score and Harris score of patients.Results:The press-needle nursing group had lower VAS score than the routine nursing group in postoperative 1 d,3 d,and 5 d.The press-needle nursing group had better sleep quality scores than the routine nursing group 3 and 5 days after operation(P<0.05).The press-needle nursing group had higher Harris score than that the routine group 1 week after operation(P<0.05).The press-needle nursing group had lower incidence of constipation,nausea and vomiting than the routine nursing group(P<0.05).Binary Logstic regression showed that press-needle nursing was the influencing factor of the severity of VAS score one day after operation(OR=0.249,95%CI=0.186-0.609,P<0.001).Multiple linear regression analysis showed that intradermal needling nursing was the influencing factor of Harris score at 1 week after operation(β=1.324,95%CI=1.201-2.668,P<0.001).Conclusion:The application of press-needle in the postoperative nursing of elderly patients with hip fracture can alleviate postoperative pain in early stage,improve hip joint function of patients and reduce the incidence of adverse reactions such as nausea,vomiting and constipation.
