宫颈提拉联合子宫下段环形蝶式缝合术在前置胎盘剖宫产术中的应用

摘　　要：目的:探究宫颈提拉联合子宫下段环形蝶式缝合术在前置胎盘剖宫产术中的应用效果。方法:2021年2月—2023年10月毕节市妇幼保健院收治的120例前置胎盘剖宫产术产妇作为研究对象,依据随机数表法将其分为对照组(低位B-Lynch缝合术)和研究组(宫颈提拉联合子宫下段环形蝶式缝合术),各60例。比较两组凝血功能、产后出血量及不良事件发生情况。结果:两组干预前凝血功能比较,差异无统计学意义(P>0.05);研究组干预后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、血浆纤维蛋白原(FIB)及血小板计数(PLT)明显低于对照组,差异有统计学意义(P<0.05)。研究组产后2 h、24 h出血量少于对照组,止血用时短于对照组,不良事件发生率低于对照组,差异有统计学意义(P<0.05)。结论:前置胎盘剖宫产术患者实施宫颈提拉联合子宫下段环形蝶式缝合术能改善凝血功能,控制产后出血量,降低不良事件发生率。Objective:To explore the application effect of cervical lifting combined with circular butterfly suturing of lower segment of uterus in cesarean section with placenta previa.Method:A total of 120 pregnant women with placenta previa undergoing cesarean section in Bijie Maternal and Child Health Hospital from February 2021 to October 2023 were selected as the research objects,they were divided into the control group(low B-Lynch suture)and the study group(cervical lifting combined with circular butterfly suturing of lower segment of uterus)according to the random number table method,with 60 cases in each group.The coagulation function,postpartum hemorrhage and adverse events were compared between two groups.Result:There were no significant differences in the coagulation function between two groups before intervention(P>0.05);the prothrombin time(PT),activated partial thromboplastin time(APTT),plasma fibrinogen(FIB)and platelet count(PLT)in the study group after intervention were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The amount of bleeding at 2 h and 24 h after delivery in the study group were less than those in the control group,the time of hemostasis was shorter than that in the control group,and the incidence of adverse events was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Cervical lifting combined with circular butterfly suturing of lower segment of uterus in patients with placenta previa undergoing cesarean section with placenta previa can improve coagulation function,control postpartum hemorrhage and reduce the incidence of adverse events.
