手术联合耻骨上区域吸脂术矫治隐匿性阴茎疗效观察

摘　　要：目的:分析手术矫正联合耻骨上区域脂肪抽吸治疗隐匿性阴茎的临床效果,探讨优化手术效果、减少手术创伤、促进术后恢复及预防并发症的对策。方法:患者分别采用隐匿性阴茎矫正术联合耻骨上区域脂肪抽吸或仅采用耻骨上区域脂肪抽吸的方法延长阴茎,对比阴茎术前、术中、术后常态下长度变化及并发症发生情况。结果:本组124例患者,耻骨上区域吸脂量75~140 ml,平均吸脂量(104.12±15.70)ml,阴茎常态下术前长度为(3.21±1.25)cm,术中完成吸脂后为(4.52±1.39)cm,矫正手术后为(5.78±0.91)cm,阴茎长度术中较术前增加(1.31±0.22)cm,术后较术前增加(2.62±0.71)cm。主要并发症为包皮水肿、皮瓣尖端坏死等。其中包皮水肿15例均在30 d左右消退,切口愈合不良11例,换药后在2~4周愈合,术后复发6例,吸脂区域血清肿3例经局部抽液加压包扎后1周痊愈。结论:隐匿性阴茎患者在常规术式矫正的同时,联合采用耻骨上脂肪抽吸术治疗效果确切,手术创伤小,术后恢复快,并发症少。Objective To analyze the clinical effect of concealed penis correction combined with suprapubic liposuction in the treatment of concealed penis,and to explore the countermeasures to optimize the surgical effect,reduce surgical trauma,promote postoperative recovery and prevent complications.Methods Patients were treated with occult penile correction surgery combined with suprapubic fat suction or only suprapubic fat suction to extend the penis.The changes in length of the penis under normal conditions before,during,and after surgery were compared,as well as the evaluation of complications.Results This group of 124 patients,on the pubic region of 75-140 ml of liposuction,liposuction average amount(104.12±15.70)ml,normal preoperative penis length is(3.21±1.25)cm,intraoperative after liposuction for(4.52±1.39)cm,after corrective surgery for(5.78±0.91)cm,The length of penis increased by(1.31±0.22)cm during operation and(2.62±0.71)cm after operation.The main complications are phimosis and necrosis of the flap tip.Among them,15 cases of phimosis edema subsided within about 30 days,11 cases of poor wound healing,healed within 2-4 weeks after dressing change,6 cases recurred after surgery,and 3 cases of serum swelling in the liposuction area were cured within 1 week after local fluid extraction and pressure bandage.Conclusion Patients with occult penis have accurate treatment effects in combination with suprapubic liposuction at the same time as conventional surgical correction,with less surgical trauma,fast postoperative recovery and few complications.
