振动排痰法联合预防性护理对ICU重症肺炎患者的护理效果分析

摘　　要：目的探讨振动排痰法联合预防性护理干预对重症监护室(ICU)重症肺炎患者的护理效果。方法选取2021年4月至2023年4月我院ICU收治的重症肺炎患者92例,采取随机数字表法分为对照组和观察组,每组46例。对照组给予常规护理,观察组给予预防性护理+振动排痰。观察比较两组患者的日排痰量、机械通气时间、住ICU时间及并发症发生情况,并评估睡眠质量和舒适度。结果干预期间,观察组的日排痰量高于对照组,差异有统计学意义(P<0.05)。观察组患者的机械通气时间、住ICU时间明显短于对照组,差异均有统计学意义(均P<0.05)。观察组的并发症发生率为4.35%低于对照组的17.39%,差异有统计学意义(P<0.05)。干预1周后,观察组的匹兹堡睡眠质量指数(PSQI)各项评分及总评分均低于对照组,差异均有统计学意义(均P<0.05)。观察组的舒适度为95.65%高于对照组的80.43%,差异具有统计学意义(P<0.05)。结论振动排痰法联合预防性护理可提高ICU重症肺炎患者的排痰量,缩短机械通气时间,减少并发症发生率,提高了患者的睡眠质量及舒适度,促进患者康复。Objective To investigate the nursing effect of vibration expectoration combined with preventive nursing intervention on severe pneumonia patients admitted to ICU.Methods A total of 92 patients with severe pneumonia admitted to ICU of our hospital from April 2021 to April 2023 were divided into 2 groups by random number table method.The control group(n=46)was given routine nursing,and the observation group(n=46)was given predictive nursing+vibration expectoration.Daily sputum output,mechanical ventilation time,ICU stay time were recorded.Sleep quality and comfort were assessed and complications observed.Results During the intervention period,the daily sputum output in the observation group was higher than that in the control group(P<0.05).The comparison of mechanical ventilation time and ICU time showed that observation group was shorter than control group(P<0.05).The total incidence of complications in the observation group was 4.35%lower than that in the control group 17.39%(P<0.05).After one week of intervention,PSQI score in observation group was lower than that in control group(P<0.05).Comfort level was 95.65%higher in observation group than 80.43%in control group(P<0.05).Conclusion Vibration expectoration combined with predictive nursing in ICU patients with severe pneumonia can effectively expectorate,promote recovery,facilitate early extubation,improve patients'sleep quality and comfort,and reduce the incidence of complications.
