摘 要:目的探讨单核细胞-高密度脂蛋白比值(MHR)、糖化血红蛋白(HbA_(1C))及血管性血友病因子(vWF)与糖尿病视网膜病变(DR)患者视网膜微血管生成的相关性。方法前瞻性选取2023年10月至2024年4月在我院就诊的200例2型糖尿病(T2DM)患者作为研究对象,根据患者是否并发DR分为两组,分别为DR组(n=60)和非DR组(n=140)。采用单因素和多因素Logistic回归分析视网膜微血管生成的相关影响因素,采用Pearson法分析两组患者的MHR、HbA_(1C)及vWF与DR的相关性。结果两组患者的T2DM病程、空腹血糖(FBG)、MHR、HbA_(1C)、vWF比较具有明显差异(均P<0.05);非DR组的MHR、HbA_(1C)、vWF均明显低于DR组(均P<0.05)。Logistic回归分析结果显示,DR组的T2DM病程、FBG均明显高于非DR组(P<0.05);T2DM病程越长、FBG水平越高,发生DR风险越大,T2DM病程长以及FBG水平高是T2DM患者并发DR的独立危险因素(P<0.05)。根据Pearson相关分析结果显示,MHR、HbA_(1C)、vWF与T2DM病程的相关系数在0.8~0.9之间,具有极强的正相关关系,MHR、HbA_(1C)、vWF与FBG的相关系数在0.5~0.7之间,具有中等程度的正相关关系。结论T2DM病程越长、FBG水平越高,发生DR风险越大,MHR、HbA_(1C)、vWF与DR患者视网膜微血管生成具有一定的正相关性,通过控制血糖水平可减轻DR患者视网膜微血管生成。Objective To investigate the correlation between monocyte-high density lipoprotein ratio(MHR),glycosylated hemoglobin(HbA1C)and von Willefibrilia factor(vWF)and retinal microangiogenesis in patients with diabetic retinopathy(DR).Methods A total of 200 patients with type 2 diabetes mellitus(T2DM)who were admitted to our hospital from October 2023 to April 2024 were prospectively selected as the study objects,and were divided into two groups according to whether they had concurrent DR,namely DR Group(n=60)and non-DR group(n=140).Univariate and multivariate Logistic regression were used to analyze the relevant influencing factors,and Pearson method was used to analyze the correlation between MHR,HbA_(1C),vWF and DR in the two groups of patients.Results There were significant differences in the course of T2DM,fasting blood glucose(FBG),MHR,HbA_(1C)and vWF between 2 groups(all P<0.05).The MHR,HbA_(1C)and vWF in the non-DR group were significantly lower than those in the DR group(all P<0.05).Logistic regression analysis showed that the duration of T2DM and FBG in DR group were significantly higher than those in non-DR group(P<0.05).The longer the course of T2DM and the higher the level of FBG,the greater the risk of DR.The longer course of T2DM and the higher level of FBG were independent risk factors for developing DR in T2DM patients(P<0.05).According to Pearson correlation analysis,the correlation coefficients of MHR,HbA_(1C),vWF and the disease course of T2DM ranged from 0.8 to 0.9,showing a strong positive correlation;the correlation coefficients of MHR,HbA_(1C),vWF and FBG ranged from 0.5 to 0.7,showing a moderate positive correlation.Conclusion The longer the course of T2DM and the higher the level of FBG,the greater the risk of DR.MHR,HbA_(1C)and vWF are positively correlated with retinal microangiogenesis in DR patients,and the control of blood glucose level can reduce retinal microangiogenesis in DR patients.
