摘 要:目的应用基于灰质骨架的统计分析(gray matter-based spatial statistics,GBSS)方法探讨新生儿期大脑皮层微结构属性发育变化。材料与方法回顾性连续纳入自2011年1月至2013年6月于我院行头颅MRI检查的73例新生儿。利用GBSS方法提取皮层骨架并将扩散张量成像(diffusion tensor imaging,DTI)参量分数各向异性(fractional anisotropy,FA)、平均扩散率(mean diffusivity,MD)投射至皮层骨架。根据新生儿T1WI脑区图谱选取双侧对称性皮层感兴趣区(region of interest,ROI)并计算额、颞、顶、枕叶皮层DTI参量平均值及每个ROI内相应参量的平均值,进一步将新生儿大脑额、颞、顶、枕叶皮层及每个ROI内DTI参量值与胎龄、校正后的出生体质量、身长、头围进行相关性分析;运用一般线性模型分别对全脑皮层DTI参量FA、MD与新生儿胎龄及出生指标的相关性进行统计分析。结果基于皮层ROI分析发现,新生儿皮层FA的变化以胎龄38周为折点,折点以前,新生儿大脑皮层FA与胎龄均无显著相关性,折点之后,顶叶皮层FA(r=0.424,P=0.009)随胎龄的增加而增加。基于48个皮层ROIs及体素水平分析中均发现,右侧颞上回(superior temporal gyrus,STG)、岛叶皮层(insular cortex,Ins)、额中回(middle frontal gyrus,MFG)、扣带回(cingular gyrus,CingG)、外侧眶额回(lateral fronto-orbital gyrus,LFOG)、额下回(inferior frontal gyrus,IFG)、中央后回(postcentral gyrus,PoCG)、中央前回(precentral gyrus,PrCG)、顶上小叶(superior parietal lobule,SPL)的FA值与胎龄呈正相关,而MD值与胎龄呈负相关(P<0.05)。右侧中央后回的FA值与胎龄相关性最高(r=0.628,P=0.032),左侧中央后回的MD值与胎龄相关性最高(r=-0.598,P=0.041)。基于体素及ROI的结果均表明出生体质量、头围、身长与新生儿皮层FA、MD参量均无显著相关性。结论新生儿大脑皮层呈现区域异质性的发育特征,初级感觉和运动皮层的发育成熟早于联合皮�Objective:To explore developmental changes in microstructural properties of the cerebral cortex during the neonatal period using gray matter-based spatial statistics(GBSS).Materials and Methods:From January 2011 to June 2013,73 neonates who underwent MRI examinations from our hospital were retrospectively recruited.The cortical skeleton was extracted and the diffusion tensor imaging(DTI)covariates fractional anisotropy(FA),and mean diffusivity(MD)were projected onto the cortical skeleton using grey matter-based spatial statistics.Bilaterally symmetrical cortical regions of interest(ROIs)were selected based on neonatal T1WI atlas of brain regions and the mean values of DTI covariates in the frontal,temporal,parietal and occipital cortex as well as the mean values of the corresponding covariates within each ROI were calculated,and the values of DTI covariates in the frontal,temporal,parietal and occipital cortex as well as within each ROI were further correlated with gestational age,corrected birth weight,crown-heel length and head circumference.The correlations between gestational age and birth indicators and cortical FA and MD parameters were statistically analyzed using a general linear model.Results:Cortical ROI-based analysis revealed that changes in neonatal cortical FA were based on a turning point of 38 weeks of gestational age.Before the turning point,there was no significant correlation between neonatal frontal,temporal,parietal,and occipital cortical FA and gestational age,and after the turning point,only the parietal cortical FA(r=0.424,P=0.009)increased with increasing gestational age.Based on all 48 cortical ROIs and voxel level analyses,FA values of right superior temporal gyrus,insular cortex,middle frontal gyrus,cingular gyrus,bilateral lateral fronto-orbital gyrus,inferior frontal gyrus,postcentral gyrus,precentral gyrus,and left superior parietal lobule were found to be positively correlated with gestational age,whereas MD values were negatively correlated with gestational age(P<0.05).The highest
