重型颅脑创伤患者的阶梯式减压结合去骨瓣减压手术治疗效果

摘　　要：目的 探讨阶梯式减压结合去骨瓣减压手术对重型颅脑创伤患者的疗效。方法 非随机选取2019年1月-2023年10月泰州市姜堰中医院收治的60例重型颅脑创伤患者为研究对象,按术式不同分为对照组(n=30,去骨瓣减压术)、观察组(n=30,阶梯式减压结合去骨瓣减压手术),比较两组的治疗效果。结果 观察组手术、住院、ICU留观时间更短,术中出血量更少,差异有统计学意义(P均<0.05)。研究组的格拉斯哥昏迷指数评分和卡氏功能指数评分为(11.12±1.42)分、(79.02±3.41)分,高于对照组的(8.22±3.24)分、(76.34±2.48)分,差异有统计学意义(t=3.132、3.481,P均<0.05)。结论 对重型颅脑创伤患者实施阶梯式减压结合去骨瓣减压手术,可有效降低颅内压,改善患者的昏迷指数及自理能力。Objective To investigate the effect of step decompression combined with decompressive craniotomy on pa-tients with severe craniocerebral trauma.Methods A total of sixty patients with severe craniocerebral trauma admitted to Jiangyan Hospital of Traditional Chinese Medicine in Taizhou City from January 2019 to October 2023 were non-randomly selected as the study subjects.They were divided into control group(n=30,decompressive craniotomy)and observation group(n=30,stepped decompression combined with decompressive craniotomy)according to different op-eration methods,and the treatment effects of the two groups were compared.Results The duration of operation,hospi-talization and ICU observation in the observation group was shorter,and the intraoperative blood loss was less,the dif-ferences were statistically significant(all P<0.05).The scores of Glasgow Coma Scale and Karnofsky Performance Sta-tus in the study group were 11.12±1.42 and 79.02±3.41,which were higher than 8.22±3.24 and 76.34±2.48 in the con-trol group,and the differences were statistically significant(t=3.132,3.481,both P<0.05).Conclusion Step decom-pression combined with craniotomy can effectively reduce intracranial pressure and improve coma index and self-care ability of patients with severe craniocerebral trauma.
