排毒保肾丸对腹膜透析患者腹膜转运效能影响的研究

摘　　要：目的:探讨腹膜透析患者采用排毒保肾丸治疗对其腹膜转运效能的临床影响。方法:从我院2021年1月—2022年6月收治的慢性肾脏病(CKD5期)行连续性不卧床腹膜透析且腹膜透析腹膜平衡试验处于低平均转运以下的患者中,抽取40例作为研究对象,根据不同的治疗方法分组,采用一般治疗的患者纳入对照组(n=20),在一般治疗基础上增加排毒保肾丸治疗的患者纳入观察组(n=20),对比2组治疗前后的转化生长因子β_(1)(TGF-β_(1))、白细胞介素-6(IL-6)、糖类抗原125(CA125)、血肌酐、血糖、肌酐透析液与血浓度之比(D/Pcr)、尿素清除率指数(k/tv)、肌酐清除率(Ccr)和超滤量。结果:治疗后,2组的TGF-β_(1)、IL-6和CA125水平均较治疗前低(P<0.05),且观察组的TGF-β_(1)、IL-6和CA125水平均较对照组低(P<0.05);2组的血肌酐水平和血糖水平均较治疗前低(P<0.05),且观察组的血肌酐水平、血糖水平均较对照组低(P<0.05);2组的D/Pcr、k/tv、Ccr和超滤量均较治疗前高(P<0.05),且观察组的D/Pcr、k/tv、Ccr和超滤量均较对照组高(P<0.05)。结论:腹膜透析患者采用排毒保肾丸治疗的效果较显著,有利于保护其残余的肾脏功能,改善微炎症反应和腹膜转运效能,提高透析充分性和超滤量,改善溶质与超滤量的平衡,延缓肾功能衰竭,临床可进一步推广。Objective:To investigate the clinical effect of Paidu Baoshen Pill on peritoneal transport efficiency in patients dialypatients.Methods:Forty patients with chronic kidney disease(CKD5 stage)admitted to our hospital from January 2021 to June 2022 who underwent continuous ambulatory peritoneal dialysis and whose peritoneal balance test was below low average transport were selected as the study subjects.According to different treatment methods,patients who received general treatment were included in the control group(n=20),while patients who received additional treatment with Paidu Baoshen Pill were included in the observation group(n=20).Comparethe transforming growth factor-β_(1)(TGF-β_(1)),interleukin-6(IL-6),carbohydrate antigen 125(CA125),serum creatinine,blood glucose,the ratio of creatinine dialysate to blood concentration(D/Pcr),urea clearance index(k/tv),creatinine clearance rate(Ccr)and ultrafiltration volume between the two groups before and after treatment.Results:After treatment,the levels of TGF-β_(1),IL-6 and CA125 in the two groups were lower than those before treatment(P<0.05),and the levels of TGF-β_(1),IL-6 and CA125 in the observation group were lower than those in the control group(P<0.05);the serum creatinine level and blood glucose level in the two groups were lower than before treatment(P<0.05),and the serum creatinine and blood glucose levels in the observation group were lower than those in the control group(P<0.05);The D/Pcr,k/tv,Ccr and ultrafiltration volume of the two groups were higher than before treatment(P<0.05).The D/Pcr,k/tv,Ccr and ultrafiltration volume of the observation group were higher than that of the control group(P<0.05).Conclusions:The effect of Paidu Baoshen Pill in the treatment of peritoneal dialysis patients is significant,which is beneficial for protecting their residual renal function,improving micro inflammatory response and peritoneal transport efficiency,increasing dialysis adequacy and ultrafiltration volume,improving the balance between solute and ultrafil
