中医理筋正骨手法配合Ponseti方法治疗低龄小儿先天性马蹄内翻足临床观察

摘　　要：目的:研究中医理筋正骨手法配合Ponseti方法治疗低龄小儿先天性马蹄内翻足的临床疗效。方法:将60例诊断为低龄小儿先天性马蹄内翻足患儿,随机分为观察组和对照组,各30例。观察组采用中医理筋正骨手法配合Ponseti方法治疗,对照组单用Ponseti方法治疗。观察和比较2组患儿的踝关节被动跖屈度、跟腱延长长度和临床疗效。结果:治疗后3、6、12、24个月,2组患儿的踝关节跖屈度、跟腱延长长度较治疗前明显改善(P<0.05),且观察组改善效果优于对照组(P<0.05)。2组在各时间段均具有临床疗效,且观察组均优于对照组(P<0.05)。结论:采用中医理筋正骨手法配合Ponseti方法治疗低龄小儿先天性马蹄内翻足,能够延长患足跟腱长度,增大患足跖屈度,矫正马蹄内翻畸形,疗效显著,值得临床推广。Objective:To study the clinical effect of the combination of traditional Chinese medicine technique of strengthening tendons and setting bones with Ponseti method in the treatment of young children with congenital equinovarus foot.Methods:60 children diagnosed with congenital equinhoe varus foot in young children were randomly divided into treatment group and control group,30 cases in each group.The treatment group was treated with Chinese medicine tensin and bone setting technique combined with Ponseti method,while the control group was treated with Ponseti method.The passive plantar flexion of ankle joint,lengthening length of Achilles tendon and clinical effect of the two groups were observed and compared.Results:At 3,6,12 and 24 months after treatment,the ankle plantar flexion and Achilles tendon elongation in the treatment group and the control group were significantly improved(P<0.05),and the improvement effect in the treatment group was better than that in the control group(P<0.05).Both the treatment group and the control group showed clinical efficacy at each time period,and the treatment group was better than the control group(P<0.05).Conclusion:The use of Chinese medicine tendon setting technique combined with Ponseti method in the treatment of young children with congenital equinhoe varus foot can increase the plantar flexion and prolong the Achilles tendon,which has a significant effect and is worth popularizing.
