摘  要:目的:探讨无创高频振荡通气(nHFOV)治疗早产儿呼吸窘迫综合征(RDS)的临床效果。方法:选取2022年1-12月广州市荔湾区妇幼保健院新生儿科收治的RDS早产儿60例作为研究对象,采用随机数字表法分为对照组和试验组,各30例。对照组采用经鼻持续气道正压通气治疗,试验组采用nHFOV治疗。比较两组血气指标、呼吸机使用时间、氧疗时间、住院时间。结果:治疗后1 h、6 h、12 h、24 h,两组pH、动脉血氧分压、氧合指数均高于治疗前,且试验组高于对照组,两组动脉血二氧化碳分压低于治疗前,且试验组低于对照组(P<0.05)。试验组呼吸机使用时间、氧疗时间、住院时间均比对照组短(P<0.05)。结论:nHFOV模式治疗RDS的临床效果显著,能够改善患儿的血气指标,缩短其呼吸机使用时间、氧疗时间及住院时间。Objective:To investigate the clinical effect of non-invasive high frequency oscillatory ventilation(nHFOV)in the treatment of premature infants with respiratory distress syndrome(RDS).Methods:A total of sixty premature infants with RDS admitted to the neonatology department of Guangzhou Liwan District Maternal and Child Health Hospital from January 2022 to December 2022 were selected as the study objects,and were divided into control group and experimental group by random number table method,with thirty cases in each group.The control group was treated with nasal continuous positive airway pressure(nCPAP),and the experimental group was treated with nHFOV.Blood gas indexes,ventilator use time,oxygen therapy time and hospital stay were compared between the two groups.Results:At 1 h,6 h,12 h and 24 h after treatment,pH,arterial partial oxygen pressure and oxygenation index in both groups were higher than before treatment,the experimental group was higher than the control group,the partial blood pressure of carbon dioxide in both groups was lower than before treatment,and the experimental group was lower than the control group(P<0.05);the ventilator use time,oxygen therapy time and hospitalization time of experimental group were shorter than those of control group(P<0.05).Conclusion:The clinical effect of nHFOV mode in the treatment of RDS is significant,which can improve the blood gas index of children,shorten the time of ventilator use,oxygen therapy and hospital stay.
