厄贝沙坦氢氯噻嗪联合美托洛尔治疗老年重症心力衰竭的临床效果分析

摘　　要：目的:分析厄贝沙坦氢氯噻嗪联合美托洛尔在重症心力衰竭(HF)老年患者中的应用效果。方法:以70例重症HF老年患者作为受试者(2022年6月-2023年6月),均为单县海吉亚医院收治,根据双盲法随机分组,每组35例,患者均为女性。对照组进行常规治疗,观察组在对照组基础上实施厄贝沙坦氢氯噻嗪联合美托洛尔联合治疗。对比两组临床疗效。结果:治疗后,两组左室舒张末期内径短于治疗前,且观察组比对照组短,两组左室射血分数、B型尿钠肽高于治疗前,且观察组比对照组高,两组6 min内步行距离长于治疗前,且观察组比对照组长(P<0.05)。治疗后,两组肿瘤坏死因子-α、超敏C反应蛋白、白细胞介素-8、白细胞介素-6、心肌肌钙蛋白Ⅰ及醛固酮水平均低于治疗前,且观察组低于对照组(P<0.05)。结论:对重症HF老年患者实施厄贝沙坦氢氯噻嗪联合美托洛尔治疗,效果较好,有助于恢复患者心功能,减轻心肌损伤,降低炎性因子水平。Objective:To analyze the effect of irbesartan hydrochlorothiazide combined with metoprolol in elderly patients with severe heart failure(HF).Methods:Seventy elderly patients with severe HF were enrolled as subjects(June 2022 to June 2023),all of whom were admitted to Shanxian County Haijiya Hospital,and were randomly assigned according to double-blind method,with thirty-five cases in each group,all of whom were female.The control group received conventional treatment,and the observation group received irbesartan hydrochlorothiazide combined with metoprolol on the basis of the control group.The clinical efficacy of the two groups was compared.Results:After treatment,the left ventricular end-diastolic diameter in both groups was shorter than before treatment,and the observation group was shorter than the control group,left ventricular ejection fraction and B-type urinary natriuretic peptide were higher in both groups than before treatment,and the observation group was higher than the control group,the walking distance within 6 min in both groups was longer than before treatment,and the observation group was longer than the control group(P<0.05);after treatment,the levels of tumor necrosis factor-α,hypersensitive C-reactive protein,interleukin-8,interleukin-6,cardiac troponinⅠand aldosterone in 2 groups were lower than before treatment,and the observation group was lower than the control group(P<0.05).Conclusion:The treatment of irbesartan hydrochlorothiazide combined with metoprolol in elderly patients with severe HF has a good effect,which is helpful to restore the heart function of patients,alleviate myocardial damage and reduce the level of inflammatory factors.
