摘　　要：目的:探讨红细胞分布宽度标准差(RDW-SD)在血液系统疾病诊断中的应用价值。方法:收集2018年12月-2023年3月首次就诊于锦州医科大学附属第三医院的190例血液系统疾病患者的临床资料,将患者分为良性疾病组(84例)和恶性疾病组(106例);收集同期锦州医科大学附属第三医院68例健康体检者的临床资料,将其作为健康对照组。收集各组RDW-SD,绘制恶性疾病组各病种受试者工作曲线,取RDW-SD最佳截断值,将恶性疾病组各病种患者分为低RDW-SD组和高RDW-SD组,比较两组患者生存差异。结果:良性血液系统疾病组和恶性血液系统疾病组RDW-SD均高于健康对照组,但良性系统疾病组RDW-SD高于恶性系统疾病组(P<0.001)。RDW-SD诊断多发性骨髓瘤、急性白血病的价值较高,对骨髓增生异常综合征有一定的诊断价值。高RDW-SD组多发性骨髓瘤、骨髓增生异常综合征及急性白血病总生存期较低RDW-SD组患者短(P<0.001)。结论:RDW-SD在血液系统疾病中的诊断价值较高,能够辅助诊断血液系统良、恶性疾病,评估患者生存期,为临床治疗和诊断提供参考。Objective:To investigate the value of standard deviation of red blood cell distribution width(RDW-SD)in the diagnosis of hematological diseases.Methods:Clinical data of one hundred and ninety patients with hematological diseases first admitted to the Third Affiliated Hospital of Jinzhou Medical University from December 2018 to March 2023 were collected,and the patients were divided into benign disease group(84 cases)and malignant disease group(106 cases).The clinical data of sixty-eight healthy subjects in the Third Affiliated Hospital of Jinzhou Medical University were collected and used as the healthy control group.The RDW-SD of each group was collected,and the working curves of subjects with different diseases in the malignant disease group were drawn,and the optimal cut-off value of RDW-SD was obtained.Patients with malignant diseases were divided into low RDW-SD group and high RDW-SD group,and the survival difference between the two groups was compared.Results:RDW-SD in benign and malignant hematological diseases group was higher than that in healthy control group,but RDW-SD in benign systemic diseases group was higher than that in malignant systemic diseases group(P<0.001);RDW-SD had high diagnostic value in multiple myeloma and acute leukemia,and had certain diagnostic value in myelodysplastic syndrome;the overall survival of multiple myeloma,myelodysplastic syndrome and acute leukemia in high RDW-SD group was shorter than that in low RDW-SD group(P<0.001).Conclusion:RDW-SD has a high diagnostic value in blood system diseases,which can assist in the diagnosis of benign and malignant diseases of blood system,evaluate the survival of patients,and provide references for clinical treatment and diagnosis.
