2型糖尿病下肢动脉病变患者应用彩色多普勒超声的诊断价值

摘　　要：目的:分析2型糖尿病下肢动脉病变患者应用彩色多普勒超声的诊断价值。方法:选取北京市西城区平安医院2022年1月-2023年6月收治的2型糖尿病下肢动脉病变患者268例作为观察组,选取同时期无2型糖尿病但患有下肢动脉病变者268例作为对照组。两组患者均行双下肢动脉超声检查,观察斑块大小、位置、形态及管腔狭窄、闭塞情况。比较两组股动脉、腘动脉、胫前动脉、胫后动脉及足背动脉血流量、血管壁内-中膜厚度(IMT)和阻力指数(RI)。结果:观察组股动脉、腘动脉、胫前动脉、胫后动脉及足背动脉血流量低于对照组,IMT、RI高于对照组。观察组下肢血管轻度病变发生率低于对照组,下肢血管中度和重度病变发生率高于对照组(P<0.001)。结论:彩色多普勒超声检查可以通过检测2型糖尿病患者的动脉血流量、IMT、RI、管腔狭窄程度,判断下肢动脉病变程度,是一种简便、准确、无创的诊断方法,可以为临床诊疗提供参考依据。Objective:To analyze the diagnostic value of color Doppler ultrasonography in type 2 diabetic patients with lower extremity arterial disease.Methods:A total of two hundred and sixty-eight patients with lower extremity arterial disease with type 2 diabetes admitted to Beijing Xicheng District Pingan Hospital from January 2022 to June 2023 were selected as the observation group,and two hundred and sixty-eight patients without type 2 diabetes but with lower extremity arterial disease were selected as the control group.Both groups underwent ultrasonography of lower extremity arteries to observe plaque size,location,shape and lumen stenosis and occlusion.Blood flow,IMT and RI of femoral artery,popliteal artery,anterior tibial artery,posterior tibial artery and dorsal foot artery were compared between two groups.Results:The blood flow of femoral artery,popliteal artery,anterior tibial artery,posterior tibial artery and dorsal pedis in observation group was lower than control group,IMT and RI were higher than control group;the incidence of mild lower extremity vascular lesions in the observation group was lower than that in the control group,and the incidence of moderate and severe lower extremity vascular lesions was higher than that in the control group(P<0.001).Conclusion:Color Doppler ultrasonography can determine the degree of lower limb artery lesions by detecting arterial blood flow,IMT,RI and lumen stenosis in patients with type 2 diabetes.It is a simple,accurate and non-invasive diagnostic method,and can provide reference for clinical diagnosis and treatment.
