基于王清任“小儿抽风不是风”理论探讨小儿慢惊风的诊疗思路

摘　　要：“小儿抽风不是风”理论是由清代医家王清任在《医林改错》中提出的有关小儿慢惊风的诊治观点,本文基于该理论和王清任的临床经验,总结提出气虚致瘀乃本病发病之根本。治疗方面依据《医林改错》中王清任创立的可保立苏汤的药物配伍特点,确立补气和血、养心安神、脾肾同补的用药思路,并对服用剂量以及时间提出了具体要求。同时,列举出预示惊风的症状,并从现代医学角度分析其合理性,以求做到“未抽先防,已抽防变”。总之,本文通过分析王清任对于小儿慢惊风发病机制的看法以及治疗时理法方药的运用,以期为临床治疗小儿慢惊风提供新的理论依据。The theory that“children's convulsions are not caused by wind”is a diagnosis and treatment view on children's chronic convulsions put forward by Wang Qingren,a doctor in the Qing Dynasty,in his book Correction of Errors in Medical Classics.Based on the theory and Wang Qingren's clinical experience,this paper proposes that Qi deficiency causing stasis is the root cause of the disease.According to the drug compatibility characteristics of Kebao Lishu decoction created by Wang Qingren in his book,this article established the treatment idea of tonifying qi,harmonizing blood,nourishing the heart,calming the mind,and tonifying spleen and kidney together,and put forward specific requirements on the dosage and time.Meanwhile,we listed the symptoms that indicate convulsions and analyzed their principles from the modern medical perspective,in order to achieve the goal of“preventing convulsions before they occur,and preventing changes after they occur.”By analyzing Wang Qingren's views on the pathogenesis of children's chronic convulsions and his application of principles and formulae in the treatment,this paper provides a new theoretical basis for clinical treatment of children's chronic convulsions.
