少腹逐瘀汤联合灌肠汤1号治疗寒湿瘀滞型盆腔炎性疾病后遗症的临床观察

摘　　要：目的:探讨少腹逐瘀汤联合灌肠汤1号治疗寒湿瘀滞型盆腔炎性疾病后遗症患者的临床效果。方法:选取2020年4月—2021年4月于迁安市中医医院妇产科就诊的60例寒湿瘀滞型盆腔炎性疾病后遗症患者,随机分为对照组和观察组,每组30例。对照组接受少腹逐瘀汤治疗,观察组在对照组治疗的基础上加用灌肠汤1号治疗。经过15 d的治疗后,比较两组患者的治疗效果、血液流变学指标、血清炎症指标、疼痛程度、包块大小、生命质量以及不良反应的发生情况。结果:治疗后,观察组患者的血液流变学参数(全血黏度、血浆黏度、纤维蛋白原、红细胞沉降率)、血清炎症标志物[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平及疼痛程度评分和包块大小均较对照组降低(P<0.05);而临床疗效及患者的生命质量评分则较对照组明显提高(P<0.05)。在治疗的过程中,两组患者均未出现明显的不良反应。结论:少腹逐瘀汤与灌肠汤1号联合治疗寒湿瘀滞型盆腔炎性疾病后遗症的效果显著,其可以有效改善患者的血液流变学和炎性介质水平,缓解患者的疼痛程度,缩小包块大小,并且具有较高的安全性。Objective:To explore the clinical effect of Shaofu Zhuyu Decoction combined with Enema Decoction No.1 in treating patients with cold-damp stagnation type pelvic inflammatory disease sequelae.Methods:Sixty patients with cold-damp stagnation type pelvic inflammatory disease sequelae who visited the obstetrics and gynecology department of Qian'an Hospital of Traditional Chinese Medicine from April 2020 to April 2021 were randomly divided into a control group and an observation group,with 30 patients in each group.The control group received treatment with Shaofu Zhuyu Decoction,while the observation group received Enema Decoction No.1 in addition to the treatment in the control group.After 15 days of treatment,the therapeutic effects,hemorheological parameters,serum inflammatory markers,pain severity,mass size,quality of life,and incidence of adverse reactions were compared between the two groups.Results:After treatment,hemorheological parameters(whole blood viscosity,plasma viscosity,fibrinogen,erythrocyte sedimentation rate),serum inflammatory markers[C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α)],pain severity scores,and mass size in the observation group were significantly lower than those in the control group(P<0.05).Furthermore,the clinical efficacy and quality of life scores of patients in the observation group were significantly higher than those in the control group(P<0.05).Throughout the treatment,no significant adverse reactions occurred in either group.Conclusion:The combination of Shaofu Zhuyu Decoction and Enema Decoction No.1 is effective in treating cold-damp stagnation type pelvic inflammatory disease sequelae.It can effectively improve patients'hemorheological and inflammatory mediator levels,alleviate pain,reduce mass size,and demonstrates high safety.
