摘  要:细菌感染是新生儿常见的疾病, 一旦发生则病情进展迅速, 短期内可并发脓毒症及脓毒症休克, 导致多脏器功能衰竭, 甚至死亡。因此, 早期诊断、及时治疗以及免疫评估均可降低新生儿细菌感染的死亡率。目前检测手段均存在一些缺陷, 急需寻找敏感且可靠的早期诊断以及评估预后的指标。CD64通常在中性粒细胞表面低表达, 随着病理因子或炎性细胞因子的刺激, 其表达显著上调。单核细胞表达的人类白细胞抗原DR(human leukocyte antigen-DR, HLA-DR)可以提示机体的免疫功能状态。因此, 中性粒细胞CD64指数和单核细胞HLA-DR可作为细菌感染及脓毒症的早期诊断和预后评估的生物标志物, 文章将对二者的生物学作用及其在新生儿细菌感染、脓毒症的临床应用进行综述。Bacterial infection is a common disease in neonates.Once it occurs,the disease progresses rapidly and may be complicated with sepsis and septic shock in a short time,leading to multiple organ failure and even death.Therefore,early diagnosis,timely treatment and immunological evaluation can reduce the mortality of neonatal bacterial infection.There are some defects in current detection methods,and it is urgent to find sensitive and reliable early diagnosis and prognostic indicators.CD64 is usually low expression on the surface of neutrophils,and its expression is significantly upregulated with stimulation of pathological factors or inflammatory cytokines.Human leukocyte antigen-DR(HLA-DR)expressed by monocytes can indicate the immune function status of the body.Therefore,neutrophil CD64 index and monocyte HLA-DR can be used as biomarkers for early diagnosis and prognosis assessment of bacterial infection and sepsis.This summarizes review the biological effects of the two indexes and their clinical application in neonatal bacterial infection and sepsis.
