摘 要:目的 观察粉尘螨滴剂舌下含服免疫治疗(SLIT)后哮喘伴变应性鼻炎儿童口呼出气一氧化氮(FeNO)及鼻呼出气一氧化氮(FnNO)值的变化,评价SLIT对哮喘伴变应性鼻炎儿童气道炎症状态的影响。方法 选取2021年10月至2022年9月于东莞东华医院就诊的60例年龄为6~12岁对尘螨过敏的哮喘伴变应性鼻炎儿童作为研究对象,随机分为对照组和试验组,每组30例。对照组患儿予规律吸入丙酸氟替卡松气雾剂,口服孟鲁司特钠咀嚼片及氯雷他定片作常规治疗,试验组患儿在此基础上配合粉尘螨滴剂SLIT规律治疗12个月,记录两组患儿治疗前与治疗3、6及12个月后的FeNO、FnNO值及临床症状分级变化,比较两组不同时期FeNO、FnNO值的差异及临床症状分级改善率的差异。结果 (1)试验组治疗3、6、12个月后FeNO、FnNO水平均低于治疗前,试验组治疗12个月后FeNO水平低于治疗3个月后;试验组治疗6、12个月后FnNO水平低于治疗3个月后、治疗12个月后FnNO水平低于治疗6个月后。(2)对照组治疗6、 12个月后FeNO、FnNO水平均低于治疗前,对照组治疗12个月后FeNO水平低于治疗前及治疗3、6个月后;对照组治疗6、12个月后FnNO水平低于治疗前及治疗3个月后。(3)两组患儿间的FeNO、FnNO水平在治疗3、6、12个月后差异均有显著性意义(P<0.05),试验组FeNO、FnNO水平均低于对照组。(4)试验组治疗6个月后哮喘控制水平分级、鼻炎发作频率的改善率均高于同期对照组(P<0.05),且试验组在治疗12个月后鼻炎发作频率改善率仍高于同期对照组(P<0.05)。结论粉尘螨滴剂SLIT能进一步降低哮喘伴变应性鼻炎儿童的FeNO及FnNO水平,改善其气道的炎症状态及临床症状。Objective To study the changes of fractional exhaled nitric oxide(FeNO) and nasal exhaled nitric oxide(FnNO) in children with asthma and allergic rhinitis after sublingual immunotherapy(SLIT) with Dermatophagoides farinae drops and evaluate the impact of SLIT on airway. Methods A total of 60 children aged 6-12 years, who were allergic to dust mites with asthma and allergic rhinitis and visited Donghua Hospital between October 2021 and September 2022, were selected as the study objects. Randomly divided the patients into a control group and an experimental group. The control group received routine treatment with Fluticasone propionate aerosol, montelukast sodium chewable tablets and loratadine tablets. The experimental group received these routine treatments along with sublingual Dermatophagoides farinae drops for 12 months. The levels of FeNO and FnNO, as well as the changes of clinical symptom scores of the children in both groups were recorded dynamically before treatment and at 3, 6 and 12 months after treatment. The differences in FeNO and FnNO levels and the improvement rate of clinical symptom scores between the two groups were compared at different time points. Results(1) In the experimental group, the FeNO and FnNO levels were lower at 3, 6 and 12 months after treatment compared to before treatment. The FeNO level was lower at 12 months compared to 3 months after treatment;the FnNO levels were lower at 6 and 12 months compared to 3 months after treatment, and the FnNO level was lower at 12 months compared to 6 months after treatment.(2) In the control group, the FeNO and FnNO levels were lower at 6 and 12 months after treatment compared to before treatment. The FeNO level at 12 months after treatment was lower than before treatment and at 3 and 6 months after treatment;the FnNO levels at 6 and 12 months after treatment were lower than before treatment and at 3 months after treatment.(3) There were significant differences in FeNO and FnNO levels at 3, 6 and 12 months after treatment between the two groups(
