摘 要:目的 系统评价2型糖尿病患者甲状腺功能的变化,为临床疾病诊断治疗提供循证医学参考。方法 检索Web of Science、PubMed、The Cochrane Library、EMbase、CBM、CNKI、万方数据库、维普数据库,检索时间为建库至2023年2月。搜集有关2型糖尿病患者甲状腺功能变化的临床研究,由2名研究者根据纳入及排除标准筛选文献,提取数据后采用RevMan 5.4软件进行Meta分析。结果 共纳入2个横断面研究、9个病例-对照研究,包括观察组3 108例2型糖尿病患者,对照组2 126例非2型糖尿病者,共计5 234例研究对象。Meta分析结果显示,2型糖尿病患者血清促甲状腺激素(thyroid stimulating hormone, TSH)值明显低于对照组(MD=1.53,95%CI:1.02~2.04,P<0.001);但观察组和对照组的血清游离三碘甲状腺原氨酸(free triiodothyronine 3,FT3)、血清游离甲状腺素(free thyroxine 4, FT4)水平差异无统计学意义(P>0.05)。结论 2型糖尿病患者甲状腺功能指标中TSH水平与对照组相比存在差异,FT3、FT4水平与2型糖尿病无相关性,建议2型糖尿病患者定期进行临床甲状腺功能检查。Objective To systematically evaluate the changes of thyroid function in patients with type 2 diabetes mellitus,and to provide evidence-based reference for clinical diagnosis and treatment.Methods The Web of Science,PubMed,The Cochrane Library,EMbase,CBM,CNKI,WanFang Data and VIP databases were searched from inception to February 2023.The clinical studies on the changes of thyroid function in patients with type 2 diabetes were collected.Two researchers screened the literature according to the inclusion and exclusion criteria.After data extraction,Meta-analysis was performed using RevMan 5.4 software.Results A total of 2 cross-sectional studies and 9 case-control studies were included,including 3108 patients with type 2 diabetes in the observation group and 2126 patients without type 2 diabetes in the control group,with a total of 5234 subjects.The results of Meta-analysis showed that the serum thyroid stimulating hormone(TSH)value in patients with type 2 diabetes was significantly lower than that in the control group(MD=1.53,95%CI:1.02~2.04,P<0.05).However,there was no significant difference in free triiodothyronine 3(FT3)and free thyroxine 4(FT4)between the two groups(P>0.05).Conclusion The TSH level in the thyroid function index of patients with type 2 diabetes mellitus is different from that of the control group.There is no correlation between FT3,FT4 levels and type 2 diabetes mellitus.It is recommended that patients with type 2 diabetes mellitus should undergo regular clinical thyroid function tests.
