骨科住院患者跌倒恐惧现状及关联因素分析

摘　　要：目的探讨骨科住院患者跌倒恐惧(FOF)的现状及其关联因素,以期为以后的研究及临床护理工作提供参考。方法对2023年1—2月东莞市某三级公立医院251例骨科住院患者进行横断面调查,调查内容包括患者的一般资料、社会支持、医学应对方式及跌倒恐惧、跌倒风险等。结果47.0%的骨科住院患者存在FOF。二元Logistic回归分析结果表明:性别(OR=2.416,95%CI:1.127~5.176,P<0.05)、最近一次发生跌倒的时间(OR=2.326,95%CI:1.154~4.689,P<0.05)、是否有慢性病(OR=2.311,95%CI:1.074~4.973,P<0.05)、是否存在跌倒风险(OR=3.587,95%CI:1.112~11.573,P<0.05)及医学应对方式(OR=0.335,95%CI:0.151~0.742,P<0.05)是骨科住院患者出现FOF的影响因素。结论骨科住院患者的FOF发生率较高,女性、最近一次发生跌倒的时间一周以上、有慢性病及存在跌倒风险是骨科住院患者FOF的独立影响因素。部分骨科患者倾向于选择回避/屈服的应对方式。护理人员在临床工作中应多关注患者的FOF情况,并制定个性化的护理措施帮助患者克服心理障碍,促进康复。Objective To explore the status and related factors of fear of falling(FOF)in orthopedic inpatients to provide reference for future research and clinical nursing.Methods A cross-sectional survey was conducted on 251 orthopedic inpatients in a Grade-Ⅲpublic hospital in Dongguan from January to February of 2023.The survey included inpatients'general information,social support,medical coping strategies,and fear of falling and risk of falling.Results 47.0%of orthopedic inpatients had FOF.The results of binary logistic regression analysis showed that gender(OR=2.416,95%CI:1.127-5.176,P<0.05),the time of the lastest fall(OR=2.326,95%CI:1.154-4.689,P<0.05),whether they have the presence of chronic diseases or not(OR=2.311,95%CI:1.074-4.973,P<0.05),whether the risk of falling exists or not(OR=3.587,95%CI:1.112-11.573,P<0.05),and medical coping strategies(OR=0.335,95%CI:0.151-0.742,P<0.05)are the influencing factors for orthopedic inpatients'FOF.Conclusion The incidence of orthopedic inpatients'FOF is relatively high,with females,the lastest fall occurred within a week or more,chronic diseases,and the risk of falling independent influencing factors for orthopedic inpatients'FOF.Some orthopedic inpatients tend to choose avoidance/surrender coping strategies.Nursing staffs should pay more attention to patients'FOF in clinical work and develop personalized nursing measures to help inpatients overcome psychological barriers and promote recovery.
