摘　　要：目的本研究旨在确定多节段脊髓型颈椎病(CSM)患者进行颈椎后路单开门椎板成形术(PCOL)时的最佳开门角度(OA)。研究方法利用颈椎三维模型模拟PCOL,计算不同OA下的椎管扩大容积和比例,以及扩容比增幅;同时在传统二维平面测量PCOL后椎管矢状径。结果随着OA的增加,椎管扩大容积和扩容比显著增加,其中0A为30°时通过三维测量得出的扩容效果最佳,而0A为60°时通过二维测量得出的扩容效果最佳。此外,0A为30°和60°时扩容比增幅和扩径比增幅最大,且差异有统计学意义(P<0.05)。结论开门角度与颈椎管扩大容积存在密切关系,通过三维测量方法计算出0A为30°时扩容效果最佳,而通过二维测量方法计算出OA为60°时扩容效果最佳。Objective To determine the optimal opening Angle(OA)for posterior cervical open-door laminoplasty(PCOL)in patients with multilevel cervical spondylotic myelopathy(CSM).Methods The three-dimensional model of cervical spine was used to simulate PCOL,and the volume and proportion of vertebral canal expansion under different OA were calculated,as well as the expansion ratio increase.At the same time,the sagittal diameter of spinal canal after PCOL was measured in traditional two-dimensional plane.Results With the increase of OA,the enlarged volume and the dilatation ratio of the spinal canal increase significantly.When OA was 30°,the dilatation effect obtained by three-dimensional measurement was the best,while when OA was 60°,the dilatation effect obtained by two-dimensional measurement was the best.In addition,when OA was 30°and 60°,the expansion ratio and expansion ratio increased the most,and had statistical significance(P<0.05).Conclusion There is a close relationship between the OA and the expansion volume of the cervical canal.The expansion effect is best when the OA is 30°calculated by three-dimensional measurement method,while the expansion effect is best when the OA is 60°calculated by two-dimensional measurement method.
