摘　　要：目的探索一种使用CO_(2)激光保甲治疗嵌甲,不损伤甲、甲母质、甲床,且操作简单的方法。方法自2021年5月15日—2023年10月13日,对16例患者共21处足第一趾嵌甲病灶采用CO_(2)激光治疗,局部麻醉下对嵌甲周围的肉芽及坏死损伤的组织进行切割、气化,显露下方健康组织,使得残甲侧缘完全暴露,达到甲沟形态重建。结果16例患者平均年龄(23.38±13.61)岁,治疗病灶21处,有2例复发(治愈率90.48%)。恢复日常行走时间为(2.25±0.66)d,恢复日常活动时间为(5.13±0.86)d。术后随访6~35个月,趾甲及侧甲廓生长良好,无疼痛及感觉异常,未见并发症。结论嵌甲可通过CO_(2)激光治疗,能保全趾甲、不需要切除甲母质及损伤甲床。病灶周围肉芽增生、感染及坏死组织的去除,能达到控制局部炎症的目的。Objective To present a new treatment for ingrown toenail with CO_(2) laser that is easy to perform,without damage the nail,nail matrix and nail bed.Methods A total of 21 ingrown toenail(IGT)lesions of the big toe in 16 patients who were diagnosed as ingrown toenail were treated with CO_(2) laser,while tissues along nail groove around the lesion was ablated,ingrown nail plate was exposed and separated.Results A total of 16 patients,with a mean age was(23.38±13.61)years.All the 21 lesions were regressed except 2 with relapse(the cure rate was 90.48%).The time of return to walking,and return to daily activity were(2.25±0.66)days and(5.13±0.86)days,respectively.During the 6-35 months follow-up,the cosmetic effect was remarkable with normal appearance and function.There were no complications.Conclusion Ingrown toenail can be surgically treated without matricectomy and preserved the nail.All necrotic tissue and certain volume of soft tissue surrounding the lesion should be ablated to decompress the nail and reduce inflammation.
