夹脊电场疗法治疗骶上脊髓损伤后神经源性膀胱尿失禁临床观察

摘　　要：目的 探讨夹脊电场疗法治疗骶上脊髓损伤后神经源性膀胱尿失禁的有效性。方法 将2019年10月—2022年10月就诊于兴化市中医医院的骶上脊髓后神经源性膀胱尿失禁患者60例,按就诊顺序随机分为治疗组和对照组,每组30例。治疗组采用骶上脊髓损伤部位上下节段夹脊电针密波联合肾俞、会阳电针疏波治疗;对照组采用膀胱功能训练法治疗。治疗2个疗程后,观察并比较2组治疗效果。结果 治疗后,2组患者残余尿量、漏尿次数、1 h尿垫试验漏尿量、SQLP评分均改善(P<0.05),治疗组优于对照组(P<0.05);治疗组总有效率高于对照组,差异具有统计学意义(P<0.05)。结论 夹脊电场疗法能显著改善骶上脊髓损伤后神经源性膀胱尿失禁患者临床症状,减少残余尿量、漏尿次数、1 h尿垫试验的漏尿量、降低SQLP量表评分,其疗效优于膀胱功能训练法治疗。Objective To explore the effectiveness of Jiaji(EX-B2) electric field therapy in the treatment of neurogenic bladder urinary incontinence after sacral spinal cord injury.Methods A total of 60 patients with neurogenic bladder incontinence after superior sacral spinal cord injury in Xinghua Hospital of Traditional Chinese Medicine from October 2019 to October 2022 were randomly divided into treatment group and control group according to the order of treatment,with 30 cases in each group.In the treatment group,the upper and lower segments of Jiaji(EX-B2) were treated with ridge electroacupuncture dense wave combined with Shenshu(BL23) and Huiyang(BL35) electroacupuncture thin wave.The control group was treated with bladder function training.After 2 courses of treatment,the therapeutic effect of the two groups was observed and compared.Results After treatment,the residual urine volume,the number of urine leakage and the 1-hour urine pad test urine leakage in 2 groups;SQLP scores were improved compared with those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).After treatment,the total effective rate of the treatment group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Jiaji(EX-B2) electric field therapy can significantly improve the clinical symptoms of patients with neurogenic bladder urinary incontinence after sacral spinal cord injury,reduce residual urine volume,leakage frequency,1-hour urine pad test leakage volume,and lower the SQLP scale score.Its curative effect is better than that of bladder function training.
