摘　　要：目的探讨连接式膨体聚四氟乙烯假体在改善鼻基底凹陷中的临床效果。方法回顾性分析自2019年4月至2023年10月,郑东蔚莱医疗美容门诊部收治的79例鼻部形态不佳伴鼻基底凹陷患者的临床资料。初次鼻整形30例,修复鼻49例。根据鼻基底凹陷程度,行鼻整形术同时从鼻小柱切口剥离腔隙置入假体。术后观察并发症,进行满意度评估,测量鼻翼基底凹陷度、鼻唇角、鼻小柱小叶角,并评价术后变化。结果79例患者切口均Ⅰ期愈合,1例术后第1天出现上唇肿胀,15 d后肿胀消失;1例术后第7天去除胶带时,发现一侧鼻基底假体稍向外移位,给予手法推挤后假体复位。满意度评价:非常满意73例,满意5例,一般1例。术后12个月,鼻翼基底凹陷度、鼻唇角、鼻小柱小叶角均较术前显著改善,差异有统计学意义(P<0.05)。结论鼻部整形同时采用连接式膨体聚四氟乙烯假体改善鼻基底凹陷效果确切,移位、不对称等并发症发生较少,满意度较高。Objective To explore the clinical efficacy of connected expanded polytetrafluoroethylene prosthesis in improving nasal base depression.Methods From April 2019 to October 2023,a total of 79 patients with poor nasal morphology with nasal base depression were retrospectively analyzed.There were 30 cases of primary rhinoplasty and 49 cases of nasal repair.According to the degree of nasal base depression,rhinoplasty was performed and the lacuna was prepared from the nasal column incision and the prosthesis was inserted.Postoperative complications were observed,patient satisfaction was assessed,and the depression of the nasal base,nasolabial angle,and columnar lobular angle were measured to evaluate the changes.Results All incisions healed by the first intention.One patient had upper lip swelling on the first day after surgery,which disappeared half a month later.One patient had a slightly outward displacement of one nasal base prosthesis in the 7th day after surgery,which was discoverd when removing the 3M tape,and the prosthesis was reduced after manual extrusion.Seventy-three patients were very satisfied,5 were satisfied and 1 was normal.The depression of the nasal base,nasolabial angle and columnar lobule angle were significantly improved compared with those before operation,with statistical significance(P<0.05).Conclusion The application of connected expanded polytetrafluoroethylene prosthesis in nasal reconstruction is effective and of high satisfactionin improving nasal base depression,with fewer complications such as displacement and asymmetry.
