摘 要:目的探讨呼吸机相关性肺炎(VAP)患者血清miR-31和LncRNA TUG1表达水平及其预后价值。方法选取本院146例VAP患者作为研究对象,根据患者30 d后生存结局将其分为生存组(n=114)与死亡组(n=32);另选取同期74例体检健康者为对照组,收集受试者临床资料,比较生存组和死亡组患者资料;采用qRT-PCR法检测各组血清miR-31和LncRNA TUG1表达水平;ENCORI网站预测miR-31和LncRNA TUG1之间是否存在靶向关系;Pearson法分析miR-31和LncRNA TUG1表达及其与TNF-α和IL-6水平的相关性;多因素Cox回归分析VAP患者预后死亡的影响因素;受试者工作特征(ROC)曲线评估血清miR-31和LncRNA TUG1表达水平对VAP患者预后死亡的评估价值。结果死亡组患者气管切开占比、二次插管占比、MV时间、TNF-α和IL-6水平均显著高于生存组(P<0.05);与对照组相比,生存和死亡VAP患者血清miR-31表达水平明显降低(1.02±0.22 vs 0.76±0.18 vs 0.51±0.14,F=89.384,P<0.05),LncRNA TUG1表达水平明显升高(1.01±0.19 vs 1.44±0.25 vs 1.78±0.34,F=125.715,P<0.05);miR-31和LncRNA TUG1存在靶向结合位点,且VAP患者血清miR-31和LncRNA TUG1表达为负相关关系(r=-0.408,P<0.05);血清miR-31与TNF-α、IL-6水平均呈负相关(P<0.05),LncRNA TUG1与TNF-α、IL-6水平均呈正相关(P<0.05);LncRNA TUG1表达水平升高、miR-31表达水平降低是VAP患者预后死亡的危险因素(P<0.05);血清miR-31和LncRNA TUG1表达水平单独及联合预测VAP的曲线下面积(AUC)分别为0.826、0.827、0.928,联合预测效果优于单独预测(Z_(二者联合-miR-31)=2.090、P=0.037,Z_(二者联合-LncRNA TUG1)=2.177,P=0.030)。结论血清miR-31和LncRNA TUG1表达水平与VAP患者预后有关,二者联合检测对VAP患者预后评估价值较高。Objective To investigate the expression levels of serum miR-31 and LncRNA TUGl in patients with ventilator-associated pneumonia(VAP)and their prognostic value.I MethodsA total of 146 VAP patients in our hospital were collected as the study subjects.They were separated into survival group(n=114)and death group(n=32)based on their survival outcomes after 30 days;meantime,74 healthy individuals who had a medical examination in our hospital were selected as a control group.Clinical data of subjects were collected;and the clinical data of patients in the survival and death groups were compared.QRT-PCR method was applied to detect the expression levels of serum miR-31 and LncRNA TUG1 in each group.ENCORI website was applied to predict whether there was a targeting relationship between miR-31 and LncRNA TUG1.Pearson method was used to analyze miR-31 and LncRNA TUG1 expression and their correlation with TNF-αand IL-6 levels.Multivariate Cox regression was applied to analyze the factors influencing the prognostic mortality of VAP patients.Receiver operating characteristic(ROC)curve was applied to evaluate the prognostic value of serum miR-31 and LncRNA TUGl expression levels for prognostic mortality in VAP patients.Results The proportion of tracheostomy,proportion of secondary intubation,MV time,TNF-αand IL-6 levels in the death group were obviously higher than those in the survival group(P<o.05).Compared with the control group,the serum miR-31 expression level in survival and dead VAP patients was significantly lower(1.02±0.22 vs 0.76±0.18 vs 0.51±0.14,F=89.384,P<0.05),and the LncRNA TUG1 expression level was significantly higher(1.01±0.19 vs 1.44±0.25 vs 1.78±0.34,F=125.715,P<0.05).MiR-31 and LncRNA TUG1 had targeted binding sites,and the expression of miR-31 and LncRNA TUG1 in the serum of VAP patients was negatively correlated(r=-0.408,P<0.05).Serum miR-31 was negatively correlated with TNF-αand IL-6 levels(P<0.05),and LncRNA TUG1 was positively correlated with TNF-αand IL-6 levels(P<0.05).Increased serum Ln
