摘 要:目的:探究补阳还五汤对失神经肌萎缩大鼠自噬的调控作用,阐明益气活血法对失神经肌萎缩的保护机制。方法:制备神经夹伤大鼠模型,实验分假手术组、模型组、弥可保组、补阳还五汤、补阳还五汤+LY294002,每组8只;各组大鼠每日灌胃给药,第18天取胫前肌计算湿重比并进行形态学观察,Western-blot检测胫前肌PI3K/PKB/mTOR(磷脂酰肌醇3-激酶/蛋白激酶/B/雷帕霉素靶蛋白)信号通路蛋白。结果:模型组大鼠胫前肌湿重比显著降低(P<0.05),而补阳还五汤组和弥可保组胫前肌湿重比显著提高(P<0.05),补阳还五汤+LY294002组胫前肌湿重比下降。形态学观察发现:模型组肌纤维横截面积减少,形态不规则,肌纤维结构紊乱,结缔组织增生较多,肌纤维明显萎缩变性。补阳还五汤组胶原纤维增生明显减少(P<0.05),形态较规则。补阳还五汤+LY294002组肌萎缩加重,胶原纤维明显增多。补阳还五汤组、弥可保组p-PI3K、p-PKB、mTOR蛋白相对表达量与模型组相比显著增加(P<0.05)。Atg5,Becline-1蛋白相对表达量显著降低(P<0.05);补阳还五汤+LY294002组p-PI3K、p-PKB、mTOR蛋白表达降低而Atg5,Becline-1蛋白表达升高。结论:益气活血法代表方——补阳还五汤可通过PI3K/PKB/mTOR通路抑制自噬-溶酶体系统过度激活,发挥失神经肌萎缩的保护作用。Objective:To explore the regulatory effect of Buyang Huanwu Decoction on autophagy in denervated muscle atrophy rats,and to elucidate the protective mechanism of Yiqi Huoxue Therapy on denervated muscle atrophy.Methods:Models of nerve crush were prepared in rats,and the experiment was divided into sham operation group,model group,Mecobalamin group,Buyang Huanwu Decoction group,Buyang Huanwu Decoction+LY294002 group,with 8 rats in each group.Each group rats were given the corresponding medicine daily,on the 18 th day,the wet weight ratio of the anterior tibialis muscle was calculated and morphological observation was performed.Western-blot was used to detect PI3K/PKB/mTOR(phosphatidylinositol 3-kinase/protein kinase/B/rapamycin target protein)signaling pathway proteins expressions.Results:The wet weight ratio of anterior tibial muscle of rats in the model group was significantly decreased(P<0.05),while it was significantly increased in the Buyang Huanwu Decoction group and Mecobalamin group(P<0.05),and it was decreased in Buyang Huanwu Decoction+LY294002 group.Morphological observation showed that the cross-sectional area of muscle fibers in the model group was decreased,the morphology was irregular,the structure of muscle fibers was disordered,connective tissue hyperplasia was increased and muscle fibers were significantly atrophic and degenerated.The proliferation of collagen fibers in Buyang Huanwu Decoction group was significantly reduced(P<0.05),and the morphology was regular.The muscle atrophy was aggravated and collagen fibers were increased significantly in the Buyang Huanwu Decoction+LY294002 group.The relative expressions of p-pi3k,p-PKB and mTOR proteins in Buyang Huanwu Decoction group and Mecobalamin group were significantly higher than those in the model group(P<0.05);the relative expression levels of ATG5 and becline-1 proteins were significantly decreased(P<0.05).The protein expressions of p-pi3k,p-PKB and mTOR were decreased in the Buyang Huanwu Decoction+LY294002 group,while the protein expressio
