个性化护理对肥胖合并阻塞性睡眠呼吸暂停低通气综合征减肥手术后干预效果

摘　　要：目的:观察个性化护理方法对肥胖合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)减肥手术后患者的干预效果及对睡眠质量的影响。方法:选取厦门大学附属第一医院普外科收治的肥胖合并OSAHS患者70例作为研究对象,采用随机数字表法分为观察组和对照组,每组35例。对照组采用常规护理方法,观察组在对照组基础上采用个性化护理。比较2组患者护理前后睡眠相关指标[呼吸暂停次数、低通气指数及匹兹堡睡眠质量指数(PSQI)评分]、嗜睡程度[Epworth嗜睡量表(ESS)评分]和鼻塞程度[视觉模拟评分法(VAS)评分],并调查2组患者护理满意度。结果:护理后,2组患者呼吸暂停次数、低通气指数、PSQI评分均较本组护理前降低(P<0.05),且观察组呼吸暂停次数、低通气指数、PSQI评分均明显低于对照组(P<0.05)。护理后,2组ESS评分及鼻塞VAS评分均较本组护理前降低(P<0.05),且观察组ESS评分及鼻塞VAS评分均明显低于对照组(P<0.05)。观察组患者护理总满意度为94.29%(33/35),对照组护理总满意度为77.14%(27/35),观察组患者护理总满意度高于对照组(P<0.05)。结论:对肥胖合并OSAHS减肥手术后患者采用个性化护理方法能够有效改善患者的睡眠质量,减轻嗜睡和鼻塞程度,提高患者满意度。Objective:To observe the effect of individualized nursing on obesity patients with obstructive sleep apnea hypopnea syndrome(OSAHS)after bariatric surgery and its effect on sleep quality.Methods:Seventy obese patients with OSAHS admitted to the Department of General Surgery of the First Affiliated Hospital of Xiamen University were selected as the study objects,and were divided into observation group and control group by random number table method,with 35 cases in each group.The control group was treated with routine nursing methods,and the observation group was treated with individualized nursing on the basis of the control group.Sleep related indexes[apnea frequency,hypopnea index and Pittsburgh Sleep Quality Index(PSQI)score],sleepiness degree[Epworth Sleepiness Scale(ESS)score]and nasal congestion degree[visual analog scale(VAS)score]were compared before and after care of patients in 2 groups,and nursing satisfaction of patients in 2 groups was investigated.Results:After nursing,the frequency of apnea,hypopnea index and PSQI score in 2 groups were lower than those before nursing(P<0.05),and the frequency of apnea,hypopnea index and PSQI score in observation group were significantly lower than those in control group(P<0.05).After care,ESS score and nasal obstruction VAS score in 2 groups were lower than those before care(P<0.05),and ESS score and nasal obstruction VAS score in observation group were significantly lower than those in control group(P<0.05).The total nursing satisfaction of the observation group was 94.29%(33/35),and that of the control group was 77.14%(27/35).The total nursing satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion:Individualized nursing for obese patients with obstructive OSAHS after bariatric surgery can effectively improve their sleep quality,reduce drowsiness and nasal congestion,and improve patient satisfaction.
