慢性鼻窦炎手术患者术腔填塞浸润布地奈德明胶海绵的临床疗效观察

摘　　要：目的:观察慢性鼻窦炎手术患者术腔填塞浸润布地奈德明胶海绵的临床疗效。方法:鼻窦炎患者共78例,均在2021年6月—2023年11月期间入贵州航天医院进行治疗。根据随机平行分组法划分为对比组与观察组,两组例数相同(n=39)。两组给予鼻内镜下鼻窦功能开放术治疗,对比组患者鼻腔填塞明胶海绵,观察组鼻腔填塞浸润布地奈德混悬液的明胶海绵。评析两组的症状评分、鼻黏液纤毛清除功能、炎性因子含量、治疗疗效、糖皮质激素并发症发生率。结果:症状评分,治疗后观察组低于对比组(P<0.05)。鼻黏液纤毛清除功能,治疗后观察组优于对比组(P<0.05)。炎性因子含量,治疗后观察组比对比组低(P<0.05)。治疗效果,治疗后观察组高于对比组(P<0.05)。糖皮质激素并发症发生率,治疗后两组对比差异无统计学意义(P>0.05)。结论:慢性鼻窦炎手术患者术腔填塞浸润布地奈德混悬液的明胶海绵,可以提高临床疗效,缓解其鼻塞症状,还可减轻炎性反应,改善鼻黏液纤毛清除功能,且安全性较佳。Objective:To observe the clinical efficacy of budesonide-infiltrated gelatin sponge packing in operative nasal cavity of patients with chronic sinusitis undergoing surgery.Methods:A total of 78 patients with sinusitis were admitted to Guizhou Aerospace Hospital for treatment from June 2021 to November 2023.According to the random parallel grouping method,the patients were divided into comparison group and observation group,with the same number of cases(n=39).Both groups received nasal endoscopic sinus function opening surgery.Comparison group was packed with gelatin sponge in the operative cavity and observation group was packed with gelatin sponge infiltrated with budesonide suspension in the operative cavity.Symptom score,nasal mucociliary clearance function,inflammatory factor content,therapeutic effect,incidence of glucocorticoid complications in the two groups were evaluated and analyzed.Results:After treatment,the symptom score in observation group was lower than that in comparison group(P<0.05).After treatment,the nasal mucociliary clearance function in observation group was better than in comparison group(P<0.05).After treatment,the content of inflammatory factors in observation group was lower than that in comparison group(P<0.05).After treatment,the therapeutic effect in observation group was higher than that in comparison group(P<0.05).There was no difference in the incidence of glucocorticoid complications between the two groups after treatment(P>0.05).Conclusion:In patients with chronic sinusitis,the packing of gelatin sponge infiltrated with budesonide suspension in the operative cavity can improve the therapeutic effect,relieve the symptoms of nasal congestion,relieve the inflammatory response,improve the nasal mucociliary clearance function,and have a good safety.
