穴位贴敷和按摩预防肛瘘术后尿潴留的临床观察

摘　　要：目的:观察穴位贴敷联合按摩预防低位单纯性肛瘘术后尿潴留的效果。方法:回顾性分析2013年1月—2022年12月苏州市中医医院肛肠科收治的200例低位单纯性肛瘘男性患者的临床资料。将手术后接受穴位贴敷联合按摩的患者纳入观察组,未接受上述治疗的患者纳入对照组。比较两组患者首次排尿状况、术后导尿情况、治疗效果、不良反应发生情况。结果:观察组首次排尿状况评分0分为75例(占比75%),评分3分为5例(占比5%)。对照组首次排尿状况评分0分为63例(占比63%),评分3分为15例(占比15%),两组比较,差异有统计学意义(P<0.05)。观察组导尿患者15例(占比15%),对照组导尿患者27例(占比27%)。两组导尿率比较,差异有统计学意义(P<0.05)。观察组总有效率(85.0%)高于对照组(73.0%),差异有统计学意义(P<0.05)。观察组患者均未发生贴敷部位皮肤过敏、水疱、烧灼等不良反应。结论:穴位贴敷联合按摩可以有效预防低位单纯性肛瘘术后男性患者尿潴留的发生。Objective:To observe the effect of acupoint application combined with massage on preventing urinary retention after operation of low simple anal fistula.Methods:The clinical data of two hundred male patients with low simple anal fistula admitted to the anorectal department of Suzhou Hospital of Traditional Chinese Medicine from January 2013 to December 2022 were retrospectively analyzed.Patients who received acupoint application combined with massage after surgery were included in the observation group,and those who did not receive the above treatment were included in the control group.The first urination,postoperative catheterization,therapeutic effect and adverse reactions were compared between the two groups.Results:In the observation group,75 cases(75%)were scored 0 and 5 cases(5%)were scored 3;in the control group,the scores of the first urination status were 0,63 cases(63%),and 3,15 cases(15%),the difference between the two groups was statistically significant(P<0.05);there were 15 patients in the observation group who underwent catheterization(accounting for 15%),and 27 patients in the control group who underwent catheterization(accounting for 27%);there was significant difference in urinary catheterization rate between the two groups(P<0.05);the total effective rate of the observation group(85.0%)was higher than that of the control group(73.0%),and the difference was statistically significant(P<0.05);no adverse reactions such as skin allergy,blister and burning occurred in the observation group.Conclusion:Acupoint application combined with massage can effectively prevent the occurrence of urinary retention in male patients after operation of low simple anal fistula.
