摘　　要：患者女,53岁,全身斑块、结节伴瘙痒40余年。皮肤科情况:头面部、躯干、四肢见苹果酱样斑块,隆起于皮面,表面高低不平,触之质软,部分斑块中央可见挛缩性瘢痕。皮损组织病理示:真皮浅层肉芽肿性炎症。结核感染γ-干扰素释放试验(TB-IGRA)(+)。诊断:多发性寻常狼疮。给予异烟肼、利福平、盐酸乙胺丁醇、吡嗪酰胺规范抗结核治疗3月后,红斑颜色变暗,斑块变平、范围缩小,部分斑块中央留有白色萎缩性瘢痕。目前患者仍在随访中。A 53-year-old female patient presented with plaques and nodules with pruritus all over the body for more than 40 years.Skin examination showed applesau-like plaques found on the head,face,trunk and limbs,which raised on the skin with uneven and soft to touch surface,and contracture scars in the center of some patches.Histological examination revealed granulomatous inflammation in the superficial dermis.Tuberculosis interferon-γrelease assay(TB-IGRA)was positive.Multiple lupus vulgaris was diagnosed.After three months of standard anti-tuberculosis treatment with isoniazid,rifampicin,ethambutol hydrochloride,and pyrazinamide,the erythema became dark,the plaque area shrank and became flat,and white atrophic scars were left in the center of some plaques.The patient is still being followed up.
