摘 要:目的:通过靶向代谢组学探讨老年特应性皮炎(AD)可能存在的发病机制及异常代谢通路。方法:将40例老年AD患者(其中18例为EOS升高AD组,22例为EOS正常AD组)作为AD组,30例健康体检者作为对照组。采用液相色谱-串联质谱(LC-MS-MS)对血浆代谢质谱进行分析;利用主成分分析法(PCA)及正交偏最小二乘-判别分析法(OPLS-DA)对AD组和对照组数据进行分析,筛选出血浆样本差异性代谢物质,通过MetaboAnalyst 5.0平台分析可能存在的异常代谢通路。结果:部分组间区分良好,筛选出差异代谢物及代谢通路。AD组与对照组间存在8种代谢差异物,3条异常代谢通路:精氨酸生物合成;丙氨酸、天冬氨酸和谷氨酸代谢;乙醛酸盐和二羧酸盐代谢。EOS升高AD组与EOS正常AD组存在5种代谢差异物,2条异常代谢通路:精氨酸生物合成;丙氨酸、天冬氨酸和谷氨酸代谢。EOS升高AD组与对照组存在5种代谢差异物,2条异常代谢通路:精氨酸生物合成;组氨酸代谢。结论:老年AD组与对照组的血浆氨基酸代谢物及代谢通路具有明显差异,可能与AD的发病机制相关。Objective:To explore the possible pathogenesis and abnormal metabolic pathways of senile atopic dermatitis(AD)by targeted metabolomics.Methods:Forty elderly AD patients(18 in the elevated EOS group and 22 in the normal EOS group)were assigned to the AD group and 30 healthy subjects to the control group.Liquid chromatography-tandem mass spectrometry(LC-MS-MS)was performed to analyze plasma metabolic MS;and the principal component analysis(PCA)and orthogonal par-tial least squares-discriminant analysis(OPLS-DA)were used to identify plasma samples and analyze possible abnormal metabolic pathways through MetaboAnalyst 5.0.Results:Some groups were well differentiated,and different metabolites and metabolic pathways were selected.AD and control groups showed differences in eight metabolites and three metabolic pathways,includ-ing arginine biosynthesis;alanine,aspartate and glutamate metabolism;glyoxylate and dicarboxylate metabolism.The elevated EOS AD group and normal EOS AD group showed differences in five metabolites and two metabolic pathways,including argi-nine biosynthesis;alanine,aspartate and glutamate metabolism.The elevated EOS group and the control group showed differ-ences in five metabolites and two metabolic pathways,including arginine biosynthesis and histidine metabolism.Conclusion:The plasma amino acid metabolites and metabolic pathways are significantly different between the elderly AD group and the con-trol group,and may be involved in the pathogenesis of AD.
