摘 要:人体下颌骨由于肿瘤、创伤、先天发育畸形等原因而施行整复手术后,下颌骨形态和关节形态必然会存在一定程度的改变,不仅会影响颌面部的形貌,还会对下颌运动模式造成改变。而下颌骨作为承载颌面部运动的刚体构件,其运动模式的改变可能导致开口度减小、前伸运动不对称等运动问题,严重者甚至会产生颞下颌关节弹响、强直等关节疾病,对语言、咀嚼等功能造成阻碍。目前下颌运动主要通过电子面弓等设备仪器进行采集分析,存在价格昂贵、操作复杂等问题。针对这一问题,利用曲率分析和微分几何原理,对基于电子计算机断层扫描(CT)图像重建的下颌骨三维模型关节窝进行曲率分析,获得功能运动的特征参数并构建运动矩阵;然后通过运动仿真、机器人夹持仿真下颌骨模拟运动实验和志愿者视觉采集进行对比实验验证。结果表明,基于曲率分析获得运动特征参数,并拟合下颌运动矩阵,其最大误差为0.45 mm,可以有效还原真实的下颌功能运动。研究结果为临床上经济便捷地诊治下颌运动疾病和颞下颌关节紊乱提供了理论基础。After the reconstructive operation of human mandible due to tumor,trauma,congenital malformation and other reasons,the shape of mandible and joint will be inevitably changed to a certain extent,leading to the change of shape and motion mode of the mandible.As a rigid body bearing motion,the change of motion mode would cause mandible motion problems such as reduction of opening degree,asymmetry of forward extension movement,and even joint diseases such as temporomandibular joint snapping and rigidity,which will hinder the function of language and chewing.At present,the mandibular motion is mainly collected and analyzed by electronic face bow and other equipment,which is expensive and complicated to operate.In order to solve this problem,curvature analysis and differential geometry principle are used to obtain functional motion feature parameters and build motion matrix for the three-dimensional mandible model socket based on computed tomography(CT)image reconstruction.Then through the motion simulation,the motion experiment by a robot clamping a physical mandible and the volunteer vision acquisition,the comparative analysis is conducted.The results show that the maximum error of the motion feature parameters obtained based on curvature analysis and the mandibular motion matrix fitting is 0.45 mm,which could effectively restore the real mandibular functional motion.The research results provide a theoretical basis for the clinical diagnosis and treatment of mandibular motion disease and temporomandibular joint disorders economically and conveniently.
