摘  要:为了明确杭州某动物园内1只小熊猫伤口感染原因,本试验采用基于下一代测序技术的宏基因组测序(mNGS)技术对伤口分泌物进行检测,并同步进行常规微生物培养和药物敏感性试验。结果显示,mNGS检测出肺炎克雷伯菌和铜绿假单胞菌等细菌以及真菌石膏样奈尼兹皮菌,同时还检测到头孢菌素耐药基因blaOXA和blaCTX-M。根据mNGS检测结果更改抗生素用药方案,用阿米卡星和氟康唑代替之前的头孢曲松进行治疗。调整用药后,小熊猫伤口愈合,细菌和真菌感染状况得到控制。相比之下,常规微生物培养分别于第3天和第7天得出细菌(肺炎克雷伯菌)和真菌(石膏样奈尼兹皮菌)培养结果,药物敏感性试验结果显示,分离的肺炎克雷伯菌对头孢菌素类耐药。2种方法检测结果基本一致。结果表明,mNGS技术可快速实现感染病原体的诊断,在野生动物感染的诊断和治疗中有较为广阔的应用前景。To determine the cause of wound infection in a red panda(Ailurus fulgens)from a zoo in Hangzhou,this study employed metagenomic next-generation sequencing(mNGS)to analyze the wound exudate.Simultaneously,conventional microbial culture and drug susceptibility tests were performed.The results from mNGS identified bacterial pathogens such as Klebsiella pneumoniae and Pseudomonas aeruginosa,as well as the fungal species Nannizzia gypsea.Additionally,cephalosporin resistance genes bla OXA and bla CTX-M were detected.Based on the mNGS results,the antibiotic treatment was adjusted from ceftriaxone to amikacin and fluconazole.After the change in medication,the Ailurus fulgens's wound healed,and the bacterial and fungal infections were controlled.In comparison,conventional microbial culture took 3 days to identify Klebsiella pneumoniae and 7 days for Nannizzia gypsea,with drug sensitivity tests revealing that the isolated Klebsiella pneumoniae was resistant to cephalosporins.The results from both methods were largely consistent.This study demonstrates that mNGS technology enables rapid diagnosis of infectious pathogens,offering broad application potential for diagnosing and treating infections in wildlife.
