清肝泻火方联合盐酸乐卡地平治疗肝阳上亢型高血压患者的效果

摘　　要：目的:观察清肝泻火方对肝阳上亢型高血压患者血压变异性及血管内皮功能的影响。方法:选取2021年5月—2023年5月南阳医学高等专科学校第二附属医院接受诊治的90例原发性高血压患者,按照随机数字表法分为治疗组和对照组,各45例。对照组予盐酸乐卡地平片治疗,治疗组在对照组治疗基础上联合清肝泻火方治疗。比较两组中医证候积分、血压变应性指标[24 h平均舒张压(24 h mDBP)、24 h平均收缩压(24 h mSBP)、日间舒张压标准差(dDSD)、日间收缩压标准差(dSSD)、夜间舒张压标准差(nDSD)、夜间收缩压标准差(nSSD)]及血管内皮功能指标[内皮素(ET)、一氧化氮(NO)、血管性血友病因子(vWF)]变化,统计2组临床疗效和不良反应发生情况。结果:治疗后,治疗组眩晕、头痛目胀、易怒急躁、口苦口干、目赤、便秘积分低于对照组(P<0.05);治疗组24 h mDBP、24 h mSBP、dDSD、dSSD、n DSD低于对照组(P<0.05);治疗组ET、vWF水平均低于对照组(P<0.05);治疗组NO水平高于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:清肝泻火方能够调节肝阳上亢型高血压患者血压变异性,改善患者血管内皮功能,有助于控制患者血压,疗效显著,且安全性较高。Objective:To observe the effect of Qinggan Xiehuo Decoction on blood pressure variability and vascular endothelial function in hypertension patients with hyperactivity of liver-yang type.Methods:A total of 90 patients with essential hypertension who were treated in the Second Affiliated Hospital of Nanyang Medical College from May 2021 to May 2023 were selected and divided into treatment group and control group according to the random number table method,45 cases in each group.The control group was treated with lercanidipine hydrochloride tablets,and the treatment group was treated with Qinggan Xiehuo decoction on the basis of the control group.The changes of TCM syndrome scores,blood pressure allergic indexes[24 h mean diastolic blood pressure(24 h mDBP),24 h mean systolic blood pressure(24 h mSBP),daytime diastolic blood pressure standard deviation(dDSD),daytime systolic blood pressure standard deviation(dSSD),nighttime diastolic blood pressure standard deviation(nDSD),nighttime systolic blood pressure standard deviation(nSSD)]and vascular endothelial function indexes[endothelin(ET),nitric oxide(NO),von Willebrand factor(vWF)]were compared between the two groups.The clinical efficacy and adverse reactions of the two groups were statistically analyzed.Results:After treatment,the scores of vertigo,headache,bloating,irritability,dry mouth,red eyes,and constipation in the treatment group were lower than those in the control group(P<0.05).The 24 h mDBP,24 h mSBP,dDSD,dSSD and nDSD in the treatment group were lower than those in the control group(P<0.05).The levels of ET and vWF in the treatment group were lower than those in the control group(P<0.05).The level of NO in the treatment group was higher than that in the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Qinggan Xiehuo decoction can regulate blood pressure variability,impr
