摘　　要：目的 对2022年浙江省湖州市某幼儿园一起急性胃肠炎疫情进行病原鉴定,并对检出的札如病毒进一步做基因分型。方法 采用多重荧光聚合酶链式反应(PCR)的方法对送检的标本进行轮状病毒、诺如病毒、札如病毒、星状病毒和肠道腺病毒的核酸检测。札如病毒阳性标本进一步采用反转录聚合酶链式反应(RT-PCR)方法对其进行RdRp区和VP1区部分片段的扩增和测序,采用系统进化树分析判定病毒基因型别。结果 送检的14份肛拭子样本和5份物表样本中检出札如病毒核酸阳性4份,其他样本均为阴性。4份阳性样本均测序成功。系统进化分析结果显示,本次疫情检出的札如病毒RdRp区和VP1区均为GⅡ.3型,未发生重组。结论 该起幼儿园急性胃肠炎疫情是由GⅡ.3型札如病毒感染所致,相关机构应加强对札如病毒的感染监测,为胃肠炎的防控提供实验室技术支持。Objective To identify the pathogen of an outbreak of acute gastroenteritis occurred in a child care setting in Huzhou,Zhejiang province,in 2022,and analyze the molecular characteristics of sapovirus isolated from the acute gastroenteritis cases in the outbreak.Methods Multiplex fluorescent polymerase chain reaction(RT-PCR)was used for the nucleic acid detections of rotavirus,norovirus,sapovirus,astrovirus and intestinal adenovirus.sapovirus positive samples were further amplified and sequenced by using reverse transcription-RT-PCR for partial fragments of RdRp and VP1 regions,and phylogenetic analysis was performed to identify the virus genotype.Results In the 14 anal swabs and 5 environmental samples submitted for the detection,4 were positive for sapovirus,while the remaining samples were all negative.4 positive samples were successfully sequenced.Phylogenetic analysis on the RdRp and VP1 regions of the strains showed that they were sapovirus GⅡ.3.Conclusion The acute gastroenteritis outbreak occurred in the child care setting was caused by infection of sapovirus GⅡ.3.It is necessary to strengthen the outbreak monitoring of sapovirus and provide laboratory technical support for the prevention and control of sapovirus gastroenteritis.
