摘　　要：目的探讨经颈外径路舌根部分切除联合腭咽成型治疗因舌咽平面阻塞导致的重度OSAHS患者手术疗效研究。方法选取因舌咽平面狭窄的重度OSAHS患者35例,经颈外径路舌根部分切除联合腭咽成形术,分析患者手术前后重要指标的改变。结果患者术后半年行PSG检查,AHI:术前64.628±19.017(次/h)、术后18.922±19.532(次/h),P值为0(P<0.01);最低血氧:术前67.114±8.927(%)、术后78.257±8.762(%),P值为0(P<0.01);会厌平面游离缘面积:术前265.217±77.543(mm^(2))、术后364.917±134.557(mm^(2))P值为0(P<0.01);会厌平面游离缘前后径:术前10.962±3.075(mm)、术后12.688±4.682(mm)P值为0.001(P<0.01);会厌平面游离缘左右径:术前24.814±6.321(mm)、术后27.748±5.400(mm)P值为0.001(P<0.01)。结论经颈外径路舌根部分切除联合腭咽成型治疗因舌咽平面阻塞导致的重度OSAHS患者疗效明显。Objective To investigate the effect of transcervical partial resection of the tongue root combined with palatopharyngoplasty in the treatment of severe OSAHS caused by obstruction of the glossopharyngeal plane.Methods Thirty five patients with severe OSAHS due to glossopharyngeal plane stenosis were selected,and the changes of important indexes before and after the operation were analyzed.Results PSG examination was performed half a year after operation.AHI:before operation 64.628±19.017(times/h)、after operation 18.922±19.532(times/h),P=0(P<0.01);The lowest blood oxygen:before operation 67.114±8.927(%)、after operation 78.257±8.762(%),P<0.01;The free edge area of epiglottic plane:before operation 265.217±77.543(mm^(2))、after operation 364.917±134.557(mm^(2)),P=0(P<0.01);The anteroposterior diameter of free edge of epiglottic plane:before operation 10.962±3.075(mm)、after operation 12.688±4.682(mm),P=0.001(P<0.01);The left and right diameter of the free edge of epiglottic plane:before operation 24.814±6.321(mm)、after operation 27.748±5.400(mm),P=0.001(P<0.01).Conclusion The treatment of severe OSAHS caused by obstruction of the glossopharyngeal plane by partial resection of the lingual root through external cervical approach combined with palatopharyngoplasty has obvious effect.
