穿刺针表面涂层对微创旋切系统穿刺力的影响

摘  要:目的乳腺微创旋切手术是治疗乳腺肿瘤的有效方式,术中需将穿刺针刺入到病灶下方进行取样,穿刺针的表面特征与穿刺过程中的穿刺力和稳定性息息相关。对穿刺针表面进行优化可以很好地降低穿刺力和穿刺力的波动,因此本文探究穿刺针的表面涂层对穿刺过程的影响。方法通过在穿刺针表面覆盖特氟龙涂层的方法,改变穿刺针的表面特征,分别制作常规针、涂层针与喷砂涂层针。其中常规针是不进行表面处理的穿刺针,涂层针为覆盖有特氟龙涂层的穿刺针,而喷砂涂层针是指在喷涂特氟龙涂层之前利用高压将磨料打到穿刺针的表面,起到增加涂层附着力的作用。实验对象选用富含脂肪的猪肉组织与富含纤维结缔的牛肉组织,使用三种穿刺针分别对两种实验对象进行多次穿刺实验,探究有无涂层的穿刺针对穿刺过程的影响;利用ANSYS仿真软件对不同表面处理的穿刺针进行受力分析。选用脂肪组织为模拟对象,通过仿真计算得到涂层针、喷砂涂层针与常规针的针体表面受力分布云图以及最大受力值。结果常规针、喷砂涂层针与涂层针穿刺力的猪肉、牛肉实验结果表明,涂层针的穿刺力(1.239 N、3.926 N)和穿刺稳定性均优于常规针与喷砂涂层针;仿真结果表明涂层针的针体受力(3.0426 MPa)明显小于常规针和喷砂涂层针。结论在穿刺针表面覆盖涂层可以有效降低穿刺力和穿刺力的波动,提高进针的稳定性。Objective Minimally invasive breast circumferential surgery is an effective way to treat breast tumors.During the operation,the puncture needle needs to be inserted below the lesion for sampling.The surface characteristics of the puncture needle are closely related to the puncture force and stability in the process of puncture.The optimization of the puncture needle surface can effectively reduce the puncture force and the fluctuation of puncture force.This study explores the influence of surface treatment of puncture needle on puncture process.Methods By covering the surface of the puncture needle with a PTFE coating,the surface characteristics of the puncture needle were changed,and conventional needles,coated needles,and sandblasted coated needles were made separately.The conventional needle was a puncture needle without surface treatment,the coated needle was a puncture needle covered with a PTFE coating,and the sandblasted coated needle refers to using high pressure to apply abrasive to the surface of the puncture needle before spraying the PTFE coating,which increased the adhesion of the coating.The experimental subjects were selected as pork tissue rich in fat and beef tissue rich in fibrous connective tissue.Three types of puncture needles were used to conduct multiple puncture experiments on the two experimental subjects to explore the effect of coated puncture needles on the puncture process.We used ANSYS simulation software to conduct force analysis on puncture needles with different surface treatments.Using adipose tissue as the simulation object,the force distribution cloud maps and maximum force values on the surface of coated needles,sandblasted coated needles,and conventional needles were obtained through simulation calculations.Results The puncture force(1.239 N,3.926 N)for coating needle was better than that for conventional and sandblast coating needle.In addition,the simulation results showed that the force of the coated needle body(3.0426 MPa)was less than that of the conventional sandblasted
