4K超高清内窥镜系统在经尿道双极等离子前列腺剜除术中的应用

摘　　要：目的探究4K超高清内窥镜系统在经尿道双极等离子前列腺剜除术(TUERP)中的应用效果。方法选取2021年2月至2023年1月在本院就诊的97例良性前列腺增生患者为研究对象,其中选择2021年2月至2022年1月行TUERP治疗的45例患者作为对照组,选择2022年2月至2023年1月在对照组基础上联合使用4K超高清内窥镜系统治疗的52例患者作为观察组。比较分析两组患者的围手术期指标、术后并发症、尿道功能与前列腺症状及生活质量情况。结果观察组患者的导尿管留置时间、住院时间及术后血红蛋白减少值均少于对照组(均P<0.05);术后两组患者的并发症发生率比较,差异无统计学意义[11.10%(5/45)vs.5.76%(3/52),P<0.05];两组术后国际前列腺症状量表(IPSS)评分、排尿后残余尿量(PVR)较术前降低,最大尿流率(Q max)及膀胱顺应性(BC)均较术前提高,且观察组患者的术后IPSS评分与PVR较对照组明显下降,Q max与BC较对照组显著增加(均P<0.05);两组患者术后的行为受限、心理社会影响与社交活动受限各项评分均较术前提高,且观察组患者术后的行为受限、心理社会影响与社交活动受限评分较对照组显著增加(均P<0.05)。结论4K超高清内窥镜系统应用于TUERP术中可改善尿道功能与前列腺症状,促进患者术后早期康复。ObjectiveTo evaluate the efficacy of 4K ultra-high definition endoscopic system in transurethral bipolar plasmakinetic enucleation of the prostate(TUERP).MethodsA total of 97 patients with benign prostatic hyperplasia from February 2021 to January 2022 in our hospital,45 patients from February 2021 to January 2022 were selected as the control group,52 patients from February 2022 to January 2023 were as the observation group.The perioperative clinical parameters,postoperative complications,urethral function,prostate symptoms and quality of life were compared between the two groups.ResultsThe catheter retention time,hospital stay and postoperative hemoglobin reduction in observation group were lower than those in control group(all P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups[11.10%(5/45)vs.5.76%(3/52),P<0.05].The scores of postoperative International Prostate Symptom Scale(IPSS),post-urination residual urine volume(PVR)was reduced compared with those before surgery,maximum urinary flow rate(Q max)and bladder compliance(BC)of patients in both groups were improved compared with those before surgery,and the IPSS scores and PVR of patients in the observation group were significantly decreased compared with those in the control group.Q max and BC were significantly increased compared with control group(all P<0.05).The scores of behavioral restriction,psychosocial impact and social activity restriction in both groups were improved after surgery,and the scores of behavioral restriction,psychosocial impact and social activity restriction in the observation group were increased more significantly than those in the control group(all P<0.05).ConclusionsThe application of 4K ultra-high definition endoscope system in TUERP can improve the function of urethra and the symptoms of prostate,promote the early postoperative rehabilitation and improve the quality of life.
