PFLAP与PFNA对老年股骨转子间骨折患者术后恢复及关节功能的影响

摘　　要：目的比较老年股骨转子间骨折(IFF)患者使用股骨近端防旋髓内钉(PFNA)与解剖型锁定钢板(PFLAP)固定的效果。方法随机将九江市第一人民医院2022年1月-2024年1月收治的80例老年IFF患者分为两组,每组各40例,PFNA组实施PFNA固定,PFLAP组实施PFLAP固定,比较两组临床指标、关节功能及并发症。结果PFNA组住院时长与术后负重时间比PFLAP组短,引流量、出血量比PFLAP组少,手术时长比PFLAP组短(P<0.05),两组骨折愈合时间比较,无统计学差异(P>0.05);两组术后Harris评分均升高,且PFNA组更高(P<0.05);PFNA组并发症发生率比PFLAP组低(P<0.05)。结论与PFLAP术比较,老年IFF患者使用PFNA固定创伤小,术后恢复更快,可提高术后关节功能,减少并发症发生。Objective To compare the efficacy of proximal femoral antirotation intramedullary nail(PFNA)and anatomic locking plate(PFLAP)fixation in elderly patients with intertrochanteric fracture(IFF).Methods 80 elderly patients with IFF admitted to the hospital from January 2022 to January 2024 were randomly divided into two groups,40 cases in each group.PFNA group received PFNA fixation and PFLAP group received PFLAP fixation.Clinical indexes,joint function and complications of the two groups were compared.Results The hospital stay and weight-bearing time in PFNA group were shorter than those in PFLAP group,the suction flow and blood loss were less than those in PFLAP group,and the operation duration was shorter than those in PFLAP group(P<0.05).There was no statistical difference in fracture healing time between the two groups(P>0.05).The Harris score was higher in both groups,and higher in PFNA group(P<0.05).The complication rate of PFNA group was lower than that of PFLAP group(P<0.05).Conclusion Compared with PFLAP,PFNA fixation in elderly IFF patients has less trauma,faster postoperative recovery,improved postoperative joint function and reduced complications.
