乙肝患者HBV-DNA、乙肝两对半及肝功能指标相关性分析

摘　　要：目的研究乙型肝炎患者病毒复制指标(HBV-DNA)、乙肝两对半及肝功能指标检测结果和关系。方法选取天津市第一医院乙肝患者286例(2022年1月-2023年10月收治)为研究对象,采用荧光定量PCR检测患者血清中的HBV-DNA水平,双抗体夹心时间分辨免疫荧光法(IFMA法)检测乙型肝炎表面抗原(HBsAg)、表面抗体(HBsAb)、e抗原(HBeAg)、e抗体(HBeAb)、核心抗体(HBcAb)水平,生化分析仪检测肝功能指标水平,对检测结果进行分析。结果286例乙肝患者两对半检测结果主要以HBsAg/HBeAg/HBcAb阳性(大三阳)和HBsAg/HBeAb/HBcAb阳性(小三阳)为主,大三阳组HBV-DNA阳性率高于其他组;HBeAg阳性组中HBV-DNA的阳性表达率明显高于阴性组(P<0.05);HBeAg阳性组中,丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、r-谷氨酰基转移酶(r-GT)、总胆汁酸(TBA)等肝功能指标水平均高于阴性组(P<0.05);在肝功能相关指标ALT、AST、TBA值上,HBV-DNA高量组>中量组>低量组(P<0.05),在r-GT值上,HBVDNA高量组>中量组>低量组;相关性分析表明,肝功能相关指标水平与HBeAg阳性、HBV-DNA载量均呈现正相关,其中,ALT、AST、TBA呈显著的正相关(P<0.05)。结论乙肝患者HBV-DNA载量、乙肝两对半及肝功能相关指标水平密切相关,联合检测可以更加客观、准确、全面地评估患者病情变化及预后情况,为疾病治疗方案的确定提供依据。Objective To study the detection results and relationship of viral replication index(HBV-DNA),HBV two-and-a-half pairs and liver function index in patients with hepatitis B.Methods 286 patients with hepatitis B(admitted from January 2022 to October 2023)in our hospital were selected as the research objects.The HBV-DNA level in the serum of patients was detected by fluorescence quantitative PCR,the hepatitis B surface antigen(HBsAg),surface antibody(HBsAb),e antigen(HBeAg),e antibody(HBeAb),core antibody(HBcAb)level was detected by double antibody sandwich time-resloved fluorrscence immunoassay(IFMA),and the liver function index level was detected by biochemical analyzer.The detection results were analyzed.Results The two-half detection results of 286 patients with hepatitis B were mainly HBsAg/HBeAg/HBcAb positive(Da Sanyang)and HBsAg/HBeAb/HBcAb positive(Xiao Sanyang),and the HBV-DNA positive rate of Da Sanyang group was higher than that of other groups.The positive expression rate of HBV-DNA in HBeAg positive group was significantly higher than that of negative group(P<0.05).In the HBeAg positive group,the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),r-glutamyltransferase(r-GT),total bile acid(TBA)and other liver function indexes were higher than those of negative group(P<0.05).In the liver function related indicators ALT,AST,TBA values,HBV-DNA high-dose group>medium-dose group>low-dose group(P<0.05),in the r-GT value,HBV-DNA high-dose group>medium-dose group>low-dose group;correlation analysis showed that the levels of liver function related indicators were positively correlated with HBeAg positive and HBV-DNA load,among which ALT,AST,TBA were significantly positively correlated(P<0.05).Conclusions HBV-DNA load,HBV two-and-a-half pairs and liver function related indicators in patients with hepatitis B are closely related.Combined detection can more objectively,accurately and comprehensively evaluate the changes and prognosis of patients,and provide a basis for the determination of tr
