冰生理盐水含漱结合自我调节理论护理用于鼻咽癌放疗患者口腔疼痛管理中的效果

摘　　要：目的研究冰生理盐水含漱结合自我调节理论护理应用于鼻咽癌放疗患者口腔疼痛管理中的应用效果。方法选择广东药科大学附属第一医院在2022年12月-2023年12月期间的80例鼻咽癌放疗患者,按照随机数字表法分成两组,对照组(n=40)实施常规护理,观察组(n=40)实施冰生理盐水含漱结合自我调节理论护理,比较两组患者的口腔黏膜炎症率、口腔黏膜放射性损伤程度,护理前后口腔舒适度评分,护理前后生活质量评分,护理前后癌性疲乏、自我效能、疼痛程度、负面情绪评分,护理满意度,不良反应发生率。结果观察组口腔黏膜炎症率、口腔黏膜放射性损伤程度低于对照组;护理前,两组患者口腔舒适度、生活质量、癌性疲乏、自我效能、疼痛程度、负面情绪评分比较,差异不显著(P>0.05),护理后,观察组上述评分均优于对照组(P<0.05);观察组护理满意度高于对照组(P<0.05);观察组不良反应发生率低于对照组(P<0.05)。结论鼻咽癌放疗患者实施冰生理盐水含漱结合自我调节理论护理,可降低口腔疼痛,价值高,值得推广。Objective To study the application effect of gargling with ice physiological saline combined with self-regulation theory nursing in the management of oral pain in patients with nasopharyngeal carcinoma undergoing radiotherapy.Methods 80 patients with nasopharyngeal carcinoma who received radiotherapy in our hospital from December 2022 to December 2023 were randomly divided into two groups.The control group(n=40)was given routine nursing care,and the observation group(n=40)was given ice saline gargling combined with self-regulation nursing care.The oral mucosal inflammation rate and the degree of oral mucosal radiation injury,oral comfort score before and after nursing;quality of life score before and after nursing;cancer fatigue,self-efficacy,pain score and negative emotion score before and after nursing;nursing satisfaction;incidence rate of adverse reactions were compared between the two groups.Results The inflammation rate of oral mucosa and the degree of radiation injury of oral mucosa in the observation group were lower than those in the control group.Before nursing,the scores of oral comfort,quality of life,cancer fatigue,self-efficacy,pain degree and negative emotion in the two groups were compared(P>0.05),and after nursing,the above scores in the observation group were better than those in the control group(P<0.05).The nursing satisfaction of the observation group was higher than that of the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion The application of ice physiological saline gargling combined with self-regulation theory nursing for patients with nasopharyngeal carcinoma undergoing radiotherapy can reduce oral pain,which is of high value and worth popularizing.
