摘　　要：目的分析大疱性类天疱疮患者血清嗜酸粒细胞趋化因子-2(eotaxin-2)、基质金属蛋白酶-9(MMP-9)水平与病情严重程度的相关性。方法选取2021年1月至2023年1月河北省沧州市人民医院收治的大疱性类天疱疮患者60例为研究组;另选取于河北省沧州市人民医院行体检的健康志愿者60例为对照组。研究组患者均采用糖皮质激素治疗,采用酶联免疫吸附试验(ELISA)检测2组受试者eotaxin-2、MMP-9水平和研究组患者BP180、BP230滴度,应用流式细胞仪检测研究组患者嗜酸粒细胞(EOS)计数水平。结果与对照组比较,研究组eotaxin-2、MMP-9水平较高(P<0.05)。与大疱性类天疱疮轻度患者比较,中度、重度患者eotaxin-2、MMP-9水平较高(P<0.05);与中度患者比较,重度患者eotaxin-2、MMP-9水平较高(P<0.05)。与轻度患者比较,中度、重度患者BP180、BP230滴度、EOS水平较高(P<0.05);与中度患者比较,重度患者BP180、BP230滴度、EOS水平较高(P<0.05)。研究组患者eotaxin-2、MMP-9与BP180、BP230、EOS均呈正相关(P<0.05)。研究组患者治疗后eotaxin-2、MMP-9水平与治疗前相比较低(P<0.05)。结论大疱性类天疱疮患者血清eotaxin-2、MMP-9水平较高,且随着病情严重程度的加重eotaxin-2、MMP-9水平升高,eotaxin-2、MMP-9均与病情严重程度相关。Objective To explore the correlation between serum eosinophilic chemotactic factor-2(eotaxin-2)and matrix metalloproteinase-9(MMP-9)levels and disease severity in patients with bullous pemphigoid.Methods Sixty patients diagnosed with bullous pemphigoid at Cangzhou People's Hospital from January 2021 to January 2023 were selected as the experimental group.Another sixty healthy volunteers from Cangzhou People's Hospital were selected as the control group.All patients in the experimental group were treated with glucocorticoids.The serum levels of eotaxin-2 and MMP-9 in both groups were quantified using enzyme-linked immunosorbent assay(ELISA),while the antibody titers of anti-BP180 and anti-BP230 were determined exclusively in the experimental group.The eosinophil(EOS)counts of patients in the experimental group were detected using flow cytometry.Results Compared to the control group,the serum levels of eotaxin-2 and MMP-9 levels in the experimental group were higher(P<0.05).Compared to mild cases of bullous pemphigoid,the serum levels of eotaxin-2 and MMP-9 were higher in moderate and severe cases(P<0.05).Compared to moderate cases,the levels of eotaxin-2 and MMP-9 were higher in severe cases(P<0.05).Compared to mild cases,the antibody titers of anti-BP180 and anti-BP230,as well as EOS counts,were higher in moderate and severe cases(P<0.05).Compared to moderate cases,the antibody titers of anti-BP180 and anti-BP230,as well as EOS counts,were higher in severe cases(P<0.05).The serum levels of eotaxin-2 and MMP-9 showed a positive correlation with BP 180,BP230,and EOS in the experimental group(P<0.05).In the experimental group,the serum levels of eotaxin-2 and MMP-9 were lower after treatment compared to before treatment(P<0.05).Conclusion The patients with bullous pemphigoid exhibited higher levels of serum eotaxin-2 and MMP-9,and the levels of both eotaxin-2 and MMP-9 were correlated with disease severity.
