老年冠心病重症监护室患者握力水平下降的影响因素分析

摘　　要：目的探讨老年冠心病重症监护室(Coronary Care Unit,CCU)患者握力水平下降的影响因素。方法选取2023年1月至2024年2月龙岩市第二医院CCU收治的86例老年冠心病患者作为研究对象,所有患者均进行握力水平测定,用单因素及多元线性回归分析患者握力水平下降的影响因素。结果所有患者握力水平为5.6~32.4 kg,平均握力水平(13.68±3.37)kg。单因素分析发现,年龄>60岁、女性、体质量指数(BMI)≤18.5 kg/m^(2)、白蛋白水平≤22 g/L、病程>10年、有吸烟史、有饮酒史、急性生理与慢性健康评分(Acute Physiology and Chronic Health EvaluationⅡ,APACHEⅡ)>22分、无抗阻训练患者的握力水平较低(P<0.05)。其中,年龄>60岁、女性、BMI≤18.5 kg/m^(2)、白蛋白水平≤22 g/L、病程>10年、APACHEⅡ评分>22分、无抗阻训练是老年CCU患者握力水平下降的独立影响因素(P<0.05)。结论老年CCU患者握力水平的下降与多种因素有关,包括年龄、性别、BMI、白蛋白水平、病程、APACHEⅡ评分以及是否进行抗阻训练等,这些因素的综合作用导致患者肌力减弱,进而影响其握力水平。Objective To investigate the factors affecting the decline of grip strength in elderly patients with coronary heart disease in Critical Care Unit(CCU).Methods From January 2023 to February 2024,86 elderly patients with coronary heart disease(CHD)in CCU were selected.The grip strength level of all patients was measured,.The influencing factors of the decline of grip strength were analyzed by univariate and multiple linear regression.Results All patients had grip strength levels ranging from 5.6 to 32.4 kg,with an average grip strength level of(13.68±3.37)kg.Univariate analysis revealed that grip strength levels were significantly lower in patients who were over 60 years old,female,had a body mass index(BMI)≤18.5 kg/m^(2),albumin levels≤22 g/L,a disease course of over 10 years,a history of smoking,a history of alcohol consumption,an APACHE II score>22,and not engaged in resistance training(P<0.05).Among these,being over 60 years old,female,having a BMI≤18.5 kg/m^(2),albumin levels≤22 g/L,a disease course of over 10 years,an APACHE II score>22,and not participating in resistance training were identified as independent factors affecting the decline in grip strength among elderly CCU patients(P<0.05).Conclusion The decline in grip strength among elderly CCU patients is associated with multiple factors,including age,gender,body mass index,albumin levels,disease course,health status scores,and engagement in resistance training.The combined effect of these factors leads to muscle weakness,thereby affecting their grip strength levels.
