血清Sirt1、TG/HDL-C、FIB及D-D水平与2型糖尿病合并糖尿病性周围神经病变的相关性

摘  要:目的探讨沉默信息调节因子1(Sirt1)、三酰甘油/高密度脂蛋白胆固醇(TG/HDL-C)、纤维蛋白原(FIB)及D-二聚体(D-D)水平与2型糖尿病(T2DM)合并糖尿病性周围神经病变(DPN)的相关性。方法回顾性抽取2022年10月至2023年5月安康市中医医院收治的T2DM患者150例,依据是否合并DPN将其分为研究组(60例,T2DM合并DPN)和对照组(90例,单纯T2DM)。研究组60例患者中DPN轻度26例,中度20例,重度14例。比较两组血清Sirt1、TG/HDL-C、FIB及D-D水平,比较研究组中不同DPN程度者血清Sirt1、TG/HDL-C、FIB及D-D水平,分析血清Sirt1、TG/HDL-C、FIB及D-D水平与T2DM合并DPN的相关性。结果研究组D-D、FIB、TG/HDL-C水平高于对照组,Sirt1水平低于对照组(P<0.05)。随着DPN程度逐渐加剧,T2DM患者Sirt1水平逐渐降低,D-D、FIB、TG/HDL-C水平逐渐升高(P<0.05)。T2DM患者DPN程度与Sirt1呈负相关,与D-D、FIB、TG/HDL-C呈正相关(P<0.05)。Logistic回归分析结果显示,D-D、FIB、TG/HDL-C为T2DM并发DPN的独立危险因素,Sirt1是T2DM并发DPN的保护因素(P<0.05)。结论Sirt1、D-D、FIB、TG/HDL-C水平与T2DM患者DPN程度密切相关,D-D、FIB、TG/HDL-C为T2DM并发DPN的独立危险因素,Sirt1是T2DM并发DPN的保护因素。Objective To investigate the correlations of the levels of silent information regulator 1(Sirt1),triglyceride/high-density lipoprotein cholesterol(TG/HDL-C),fibrinogen(FIB),and D-dimer(D-D)with diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 150 patients with T2DM admitted to Ankang Traditional Chinese Medicine Hospital from October 2022 to May 2023 were retrospectively selected,and they were divided into a study group(60 cases,T2DM complicated by DPN)and a control group(90 cases,T2DM alone)according to occurrence of DPN.Among the 60 patients in the study group,26 cases had mild DPN,20 cases had moderate DPN,and 14 cases had severe DPN.The serum levels of Sirt1,TG/HDL-C,FIB and D-dimer were compared between the two groups.The serum levels of Sirt1,TG/HDL-C,FIB and D-dimer in patients with different DPN degrees in the study group were compared.The correlations of serum levels of Sirt1,TG/HDL-C,FIB and D-D with T2DM complicated by DPN were analyzed.Results The levels of D-D,FIB and TG/HDL-C in the study group were higher than those in the control group,and the level of Sirt1 in the study group was lower than that in the control group(P<0.05).With the gradual aggravation of DPN,the level of Sirt1 in T2DM patients gradually decreased,and the levels of D-D,FIB,TG/HDL-C gradually increased(P<0.05).The degree of DPN in patients with T2DM was negatively correlated with Sirt1,and was positively correlated with D-D,FIB,TG/HDL-C(P<0.05).Results of logistic regression analysis showed that D-D,FIB and TG/HDL-C were independent risk factors for T2DM complicated by DPN,moreover,Sirt1 was a protective factor for T2DM complicated by DPN(P<0.05).Conclusions The levels of Sirt1,D-D,FIB,TG/HDL-C are closely related to the degree of DPN in patients with T2DM.D-D,FIB,and TG/HDL-C are independent risk factors for DPN in patients with T2DM,while Sirt1 is the protective factor for DPN in T2DM.
