摘 要:目的探讨四维超声成像联合颈项透明层(NT)检查在产前胎儿体表畸形筛查中的应用效果。方法回顾性抽取2021年1月至2023年1月至鹤壁市妇幼保健院建档产检的1000例孕妇作为研究对象,所有孕妇于孕11~14周接受NT检查,于孕20~24周接受四维超声成像检查,以胎儿出生或引产后结果作为"金标准",探讨四维超声成像联合NT检查在产前胎儿体表畸形筛查中的价值。结果经胎儿娩出或引产结果确诊,1000例进行产前体表畸形筛查的孕妇中53例胎儿存在体表畸形(5.30%,53/1000),947例胎儿无体表畸形(94.70%,947/1000)。四维超声成像、NT检查单独评估产前胎儿体表畸形结果与金标准的结果一致性一般(Kappa=0.724、0.635);四维超声成像联合NT检查评估产前胎儿体表畸形与金标准的一致性好(Kappa=0.848)。联合检查评估产前胎儿体表畸形的准确度、灵敏度均高于四维超声成像、NT单独检查,差异有统计学意义(P<0.05)。三种方法评估产前胎儿体表畸形的特异度比较,差异未见统计学意义(P>0.05)。结论四维超声成像联合NT检查对胎儿体表畸形评估的准确度、灵敏度较高,可为产前胎儿体表畸形筛查提供帮助。Objective To investigate the application effect of four-dimensional ultrasound imaging combined with nuchal translucency(NT)examination in prenatal screening for fetal superficial deformities.Methods A total of 1000 pregnant women who completed registration and underwent prenatal checkups in Hebi Maternal and Child Health Hospital from January 2021 to January 2023 were selected as research objects retrospectively.They all underwent NT examination at 11-14 weeks of pregnancy,and completed four-dimensional ultrasound imaging examination at 20-24 weeks of pregnancy.The results of fetal birth or induced labor were used as the gold standard to investigate the value of four-dimensional ultrasound imaging combined with NT examination in prenatal screening for fetal superficial deformities.Results According to the confirmed results of fetal delivery or induction,53 of the 1000 pregnant women who underwent prenatal screening for superficial deformities had superficial deformities(5.30%,53/1000),and 947 fetuses had no superficial deformities(94.70%,947/1000).Four-dimensional ultrasound imaging and NT examination had general consistencies with gold standard in evaluating fetal superficial deformities(Kappa=0.724,0.635).The combination of four-dimensional ultrasound imaging and NT examination showed good consistency with the gold standard in evaluating fetal superficial deformities(Kappa=0.848).The combined evaluation had a higher accuracy and sensitivity in predicting fetal superficial deformities compared with four-dimensional ultrasound imaging alone and NT examination alone(P<0.05).There was no statistical significant difference in the specificity of three methods in evaluating fetal superficial deformities(P>0.05).Conclusions The combination of four-dimensional ultrasound imaging and NT examination has a high accuracy and sensitivity in evaluating fetal superficial deformities,and can provide reference for prenatal screening for fetal superficial deformities.
