老年2型糖尿病患者脑白质病变严重程度相关因素分析

摘　　要：目的探讨老年2型糖尿病(T2DM)患者脑白质病变进展的相关因素。方法选取华润武钢总医院2020年1月-2023年1月在神经科就诊的老年T2DM合并脑白质病变患者87例。将患者分为未进展组(55例)和进展组(32例)。收集并记录患者的基础信息包括性别、年龄、受教育年限、糖尿病病程、既往病史,包括高血压史等个人信息及实验室指标甘油三酯-葡萄糖指数(TyG)、同型半胱氨酸(Hcy)、肝肾功能、血脂、血糖、糖化血红蛋白(HbA1c)水平。采用多因素Logistic回归分析探究老年T2DM患者脑白质病变进展的影响因素。结果两组患者性别、是否患有高血压、受教育程度、糖尿病病程、HbA1c、血糖、尿酸(UA)、HCY差异均无统计学意义(P>0.05)。进展组患者年龄、胆固醇、甘油三酯、低密度脂蛋白、病史者数较未进展组明显升高,进展组高密度脂蛋白病史数较未进展组低,差异均有统计学意义(P<0.05)。采用二元Logistics回归方法,校正患者年龄、胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白水平,分析结果显示,老年T2DM患者脑白质病变进展与年龄、TyG指数相关(P<0.05)。结论年龄、TyG指数是老年T2DM患者脑白质病变进展发生的独立危险。Objective To explore the factors related to the progression of leukodystrophy in elderly patients with type 2 diabete.Methods A total of 87 elderly T2DM patients with leukoencephalopathy were selected from the Department of Neurology of China Resources Wisco General Hospital from January 2020 to January 2023.The patients were divided into two groups:no progression group(55 cases)and progression group(32 cases).Basic information of the patients was collected and recorded,including gender,age,years of education,course of diabetes,previous medical history,including history of hypertension,personal information,and laboratory indicators such as triglyceride-glucose index(TyG),homocysteine(Hcy),liver and kidney function,blood lipid,blood glucose,and glycosylated hemoglobin(HbA1c).Multivariate Logistic regression analysis was used to investigate the factors affecting the progression of leukinopathy in elderly patients with T2DM.Results There were no significant differences in gender,hypertension,education level,diabetes course,HbA1c,blood glucose,uric acid(UA)and HCy between 2 groups(P>0.05).The number of patients with history of age,cholesterol,triglyceride,low density lipoprotein and high density lipoprotein in the progressive group were significantly higher than those in the non-progressive group,with statistical significance(P<0.05).Binary Logistics regression method was adopted to adjust the levels of age,triglyceride,cholesterol,triglyceride,low density lipoprotein and high density lipoprotein.The results showed that the progression of leucomatosis in elderly patients with type 2 diabetes was significantly correlated with age and Tgy index(P<0.05).Conclusion Age and TyG index are independent risk factors for progression of leukodystrophy in elderly patients with type 2 diabetes.
