2型糖尿病患者血清IGF-1、IGFBP-3表达与甲状腺功能减退的相关性

摘　　要：目的探讨2型糖尿病(T2DM)患者胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白-3(IGFBP-3)表达与甲状腺功能减退的相关性。方法选取2022年1月-2023年3月在本院就诊的单纯T2DM患者63例为T2DM组,并选取同期T2DM伴甲状腺功能减退患者60例为研究组以及健康体检者60例为健康组。收集受试者一般资料和生化指标,酶联免疫吸附法检测血清IGF-1、IGFBP-3水平,化学发光法检测游离三碘甲状腺原氨酸(FT 3)、游离甲状腺素(FT 4)、促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TgAb)。采用Pearson相关系数分析IGF-1、IGFBP-3与其他指标相关性;采用Logistic回归分析T2DM患者甲状腺功能减退的影响因素。结果研究组、T2DM组BMI、空腹血糖(FPG)、餐后两小时血糖(2 h PG)、空腹胰岛素(FINS)、糖化血红蛋白(HbAlc)、TC、TG、LDL、Cr、BUN、血尿酸(UA)、TSH、甲状腺过氧化物酶抗体(TPOAb)、抗甲状腺球蛋白抗体(TgAb)显著高于健康组,HDL、FT 3、FT 4、IGF-1、IGFBP-3低于健康组(均P<0.05);研究组FPG、HbAlc、TC、TG、LDL、Cr、UA、TSH、TPOAb、TgAb显著高于T2DM组,HDL、FT 3、FT 4、IGF-1、IGFBP-3低于T2DM组(均P<0.05)。研究组血清IGF-1、IGFBP-3与FPG、FINS、HbAlc、TC、TSH、TgAb呈负相关(P<0.05)。FPG、HbAlc、TC、TSH、IGF-1、IGFBP-3是T2DM患者甲状腺功能减退发生的影响因素(P<0.05)。结论T2DM伴甲状腺功能减退患者血清IGF-1、IGFBP-3水平异常降低,二者与甲状腺功能减退的发生密切相关。Objective To investigate the relationship between the expression of insulin-like growth factor-1(IGF-1),insulin-like growth factor binding protein-3(IGFBP-3)and hypothyroidism in patients with type 2 diabetes mellitus(T2DM).Methods A total of 63 patients with simple T2DM who visited our hospital from January 2022 to March 2023 were collected as the T2DM group,60 patients with T2DM accompanied by hypothyroidism during the same period were selected as the study group,and 60 healthy individuals were regarded as the health group.The general information and biochemical indicators of the subjects were collected,enzyme-linked immunosorbent assay was applied to detect serum levels of IGF-1 and IGFBP-3,chemiluminescence was applied to detect free triiodothyronine(FT 3),free thyroxine(FT 4),thyroid hormone(TSH),thyroid peroxidase antibody(TPOAb),anti-thyroid autoantibodies(TgAb).Pearson correlation coefficient was applied to analyze the correlation between IGF-1,IGFBP-3 and other indicators;Logistic regression was applied to analyze the influencing factors of hypothyroidism in T2DM patients.Results BMI,FPG,2h PG,FINS,HbAlc,TC,TG,LDL,Cr,BUN,UA,TSH,TPOAb,TgAb in the study group and T2DM group were obviously higher than those in the healthy group,while HDL,FT 3,FT 4,IGF-1,and IGFBP-3 were lower than those in the healthy group(P<0.05);the levels of FPG,HbAlc,TC,TG,LDL,BUN,UA,TSH,TPOAb,and TgAb in the study group were obviously higher than those in the T2DM group,while levels of HDL,FT 3,FT 4,IGF-1,and IGFBP-3 were lower than those in the T2DM group(P<0.05).The serum IGF-1 and IGFBP-3 in the study group were negatively correlated with FPG,FINS,HbAlc,TC,TSH,and TgAb(P<0.05).FPG,HbAlc,TC,TSH,IGF-1,and IGFBP-3 were influencing factors for the occurrence of hypothyroidism in T2DM patients(P<0.05).Conclusion Patients with T2DM and hypothyroidism have abnormally low levels of serum IGF-1 and IGFBP-3,which are closely related to the occurrence of hypothyroidism.
