摘 要:目的探讨IL-10-1082G/A基因多态性与急性呼吸窘迫综合征(ARDS)风险的相关性。方法选取2022年1月-2022年12月本院急诊医学科收治的ARDS患者83例为ARDS组,同时选取本院体检健康的受试者80例作为对照组。收集受试者的空腹静脉血进行DNA提取,针对IL-10-1082G/A基因位点进行PCR扩增,采用ABI 3730XL遗传分析仪进行Sanger测序,确定IL-10-1082A/G的基因型和等位基因频率。结果IL-10-1082A/G显示了一个390 bp的基因片段和249 bp、196 bp的等位基因特异性片段,IL-10-1082A/G基因型包括野生纯合型AA基因型、突变纯合型GG基因型和突变杂合型AG基因型;对照组和ARDS组的IL-10-1082A/G位点各基因型符合Hardy-Weinberg平衡定律;与对照组相比,ARDS组IL-10-1082A/G位点AG、GG及G等位基因频率显著升高(P<0.05);IL-10-1082A/G位点AG基因型人群较AA型患者ARDS易感性增加(OR=2.97,95%CI=1.09~6.53,P=0.015),G等位基因型ARDS易感性增加(OR=2.16,95%CI=1.01~6.93,P=0.024)。纳入了性别、年龄、BMI、吸烟史等调整因素进行多因素Logistic分析,结果发现,IL-10-1082A/G位点AG基因型(OR=3.13,95%CI=1.19~4.52,P=0.009)和G等位基因型(OR=2.39,95%CI=1.42~5.87,P=0.036)ARDS易感性也增加。结论IL-10-1082A/G位点多态性与ARDS的易感性有关,尤其是共显性IL-10-1082A/G位点AG基因型和G等位基因可增加ARDS的发病风险。Objective To investigate the correlation between IL-10-1082G/A gene polymorphism and the risk of acute respiratory distress syndrome.Methods 83 cases of ARDS patients admitted to intensive care Department of our hospital from January 2022 to December 2022 were selected as ARDS group,and 80 healthy subjects were selected as control group.Fasting venous blood of the subjects was collected for DNA extraction,and the IL-10-1082G/A gene locus was amplified by PCR.ABI 3730XL genetic analyzer was used for Sanger sequencing to determine the genotype and allele frequency of IL-10-1082A/G.Results IL-10-1082A/G showed a 390bp gene fragment and allele-specific fragments of 249bp and 196bp.The genotypes of IL-10-1082A/G included wild homozygous AA genotype,mutant homozygous GG genotype and mutant heterozygous AG genotype.The observed values and expected values of IL-10-1082A/G genotypes in the control group and ARDS group wereχ2 test,which was consistent with Hardy-Weinberg equilibrium law.Compared with the control group,the frequencies of AG,GG and G alleles at IL-10-1082A/G sites in ARDS group were significantly increased(P<0.05).AG genotype at IL-10-1082A/G was more susceptible to ARDS than AA patients(OR=2.97,95%CI=1.09-6.53,P=0.015).The susceptibility of allele G to ARDS was increased(OR=2.16,95%CI=1.01-6.93,P=0.024).Adjusted factors such as gender,age,BMI and smoking history were included in the multivariate Logistic analysis,and the results showed that,The AG genotype(OR=3.13,95%CI=1.19-4.52,P=0.009)and the G allele(OR=2.39,95%CI=1.42-5.87,P=0.036)at the IL-10-1082A/G locus also increased susceptibility to ARDS.Conclusion The polymorphism of IL-10-1082A/G site is related to the susceptibility of ARDS,especially the AG genotype and G allele of co-dominant IL-10-1082A/G site may increase the risk of ARDS.
